摘  要:目的观察支气管哮喘患儿急性发作期T淋巴细胞亚群水平变化及IL-27对Th细胞的调节作用,探讨其与哮喘发病的关系。方法选择10例哮喘患儿(哮喘组)和5例健康儿童(健康对照组)为研究对象。流式细胞仪检测其外周静脉血T淋巴细胞亚群(CD4+T、CD8+T、CD4+T/CD8+T);收集其外周血单个核细胞(PBMC),加入不同水平(5.0μg.L-1和0.5μg.L-1)重组人IL-27(rhIL-27)进行体外培养12h,ELISA法测定其培养上清液中IFN-γ、IL-4水平。结果哮喘组CD4+T淋巴细胞,CD4+/CD8+[(38.408±3.808)%,1.480±0.155]均显著高于健康对照组[(28.240±1.936)%,1.151±0.092](Pa<0.05),CD3+[(60.354±4.770)%]、CD8+T淋巴细胞[(26.080±2.592)%]与健康对照组[(61.978±4.020)%,(24.706±2.753)%]比较差异均无统计学意义(Pa>0.05);不同水平的rhIL-27刺激PBMC进行体外培养后,IFN-γ表达[(116.160±54.890)μg.L-1,(9.369±6.910)μg.L-1]较健康对照组[(7.844±8.380)μg.L-1]均显著升高(Pa<0.005);高水平rhIL-27(5.0μg.L-1)组IL-4水平[(12.600±2.433)μg.L-1]较健康对照组[(70.820±22.912)μg.L-1]明显降低(P<0.01),而低水平rhIL-27(0.5μg.L-1)组IL-4水平[(81.480±6.396)μg.L-1]与健康对照组[(70.820±22.912)μg.L-1]比较差异无统计学意义(P>0.05)。结论 CD4+T淋巴细胞(Th细胞)在哮喘的炎症反应中起重要作用,且IL-27可调控Th1/Th2细胞,为哮喘治疗提供一种新的途径。Objective To investigate the changes of T lymphocyte subsets in asthmatic children and effect of IL - 27 on cytokine production of helper T lymphocyte subsets (Thl/Th2). Methods The subsets of T lymphocyte in 10 cases of asthmatic children ( asthmatic group) in attacking stage and 5 healthy children (healthy control group)were measured by flow cytometry. PBMCs from 10 asthmatic children were cultured with recombinant human IL -27 (5 ug ~ L^-1 or 0. 5 ug ~ L^-1 ) for 12 h in vitro, and then the levels of IFN and IL -4 in their supernatant were measured by ELISA. Results The levels of CD4 + T cell [ (38. 408 ± 3. 808) % ] and ratio of CD4+/CD8+ T cell ( 1. 480 ± 0. 155 ) in asthmatic children were significantly increased than those in healthy control group [ (28. 240 ± 1. 936) % , 1.151 ± 0. 092 ] ( P 〈 0.05 ) , but the levels of CD3 + T cell[ (60. 354 ± 4. 770) % ] and CD8 + T cell[ (26. 080 ± 2. 592) % ] in asthmatic group had no remarkable difference compared with healthy control group[ (61. 978 ± 4. 020)% , (24. 706 ±2. 753 )% ] (P 〉 0.05 ). PBMCs from children were divided into 3 groups ,5.0ug·L^-1 IL-27 group, 0. 5ug·L^-1 IL-27 group and the healthy control group. The values of IFN - γ, in supernatant were (116.160±54.89)ug·L^-1,(9.369±6.910) ug·L^-1 and (7.844±8.380) ug·L^-1, respectively. The levels of IFN-γafter stimulation with IL - 27 were remarkably higher than those in healthy control group ( P〈 0. 005 ). The levels of IL-4 in 5.0ug·L^-1 IL - 27 group ,0.5 ug·L^-1 IL- 27 group and healthy control group were (12. 600 ± 2. 433 )ug·L^-1, (81. 480±6. 396) ug·L^-1 and (70. 820 ± 22.912)ug·L^-1 ,respectively. The levels of IL-4 in 5 ug·L^-1 IL-27 group had a significant decrease compared with that in healthy control group(P 〈0.01 ) ,but the value of IL-4 in 0.5 ug·L^-1 IL- 27 group had no significant difference compared with that in healthy control group ( P 〉 0.05 ).
