摘　　要：目的:探讨cystatin C在慢性肾脏病(CRD)病人肾功能评估中的诊断效能,为临床应用提供更多的理论依据。方法:选择CRD病人151例为研究对象,正常人群40人为对照。所有入选者均测定血cystatin C、血BUN、血SCr,及血α1-MG、β2-MG,算出CCR并按身高,体重校正。以简化MDRD公式计算所得GFR为诊断标准,评价cystatin C与其他指标相比评估GFR的价值。结果:相关系数的绝对值显示cystatin C最高,cystatin C、BUN、SCr、CCR与GFR的相关性高于α1、β2-MG与GFR的相关性。各指标的AUC ROC以cystatin C的值最大。结论:(1)cystatin C可以作为评估CRD病人GFR的内源性标志物。(2)cystatin C能准确反映CRD病人肾小球滤过功能,尤其能敏感提示早期肾功能损害。Objective：To explore the diagnostic efficiency of cystatin C for renal function evaluation of chronic renal disease（CRD） patients so that to provide further theoretical basis for clinical application.Methods：151 CRD patients were selected as study objects.40 normal persons were selected as control.Blood cystatin C,blood BUN,blood SCr and blood α1-MG,β2-MG of all selected persons were tested.CCR was calculated and corrected based on stature and body weight.Simplified MDRD formula was used to obtain GFR.The value of GFR was evaluated by comparing cystatin C with other indexes.Results：The absolute value of correlation coefficient of cystatin C was the greatest.The correlation of cystatin C,BUN,SCr,CCR with GFR was greater than that of α1-MG,β2-MG.The AUCROC value of cystatin C was the greatest among all indices.Conclusions：（1）Cystatin C can be used as an endogenetic indicator to evaluate the GFR of CRD patients.（2）Cystatin C can accurately reflect the glomerular filtration function of CRD patients,especially it can sensitively indicate early renal function impairment.
