紫杉醇所致过敏性休克患者再用紫杉醇1例报告

摘　　要：1例70岁女性患者,卵巢癌术后21d,给予紫杉醇210mg加入0.9%氯化钠注射液500ml静脉滴注化疗。5min后患者出现胸闷、全身针刺感,随后出现意识丧失,面色苍白,四肢湿冷,呼吸困难,BP80/62mmHg,HR100次/min。立即停用紫杉醇,给予地塞米松及盐酸异丙嗪,10min后症状缓解。之后,再次低浓度、慢速滴注紫杉醇,未再发生过敏反应。A 70-year-old woman received an IV infusion of paclitaxel 210 mg in 0.9% sodium chloride 500 ml for chemotherapy 21 days after ovarian cancer operation. Five minutes later,the patient developed chest distress and generalized acanthesthesia,and then unconsciousness,pallor,clamminess in her extremities,dyspnea occurred. Meanwhile her blood pressure was 80/62 mm Hg and heart rate was 100 beats/min. Paclitaxel was discontinued immediately and dexamethasone and promethazine hydrochloride were given. Ten minutes later,her symptoms relieved. Subsequently,anaphylactic reactions did not recur after reexposure to a low concentration paclitaxel infusion at a slow rate.
