表现为匍行性回状红斑的类风湿性嗜中性皮炎1例

摘　　要：患者女,75岁。躯干及四肢皮疹6月,伴轻痒或轻压痛;皮损表现为匍行性回状红斑样。类风湿因子5094.5u/mL,抗环瓜氨酸抗体阳性。皮损组织病理示:棘层轻度水肿,其上方可见Kogoj脓疱,真皮全层小血管周围中度散在以淋巴细胞、中性粒细胞为主的炎性细胞浸润,伴明显的核碎裂。诊断:类风湿性嗜中性皮炎。A 75-year-old woman had a 6-month history of Erythema Gyratum Repens on her trunk and extremities. Rheumatoid factor was 4 099u/mL, anti-cyclic citrullinated antibodies was positive. Lesions histopathology showed： acanthosis edema, which can be seen above Kogoj pustules, dermal full-thickness moderate scattered around the small blood vessels to lymphocytes neutrophil-based inflammatory cell infiltration, with significant nuclear fragmentation. Diagnosin： rheumatoid neutrophilic dermatitis. The diagnosis was established by history, clinical features, laboratory examination.
