摘 要:目的:观察二种不同浓度钙离子透析液对终末期肾病(end-stage renal disease,ESRD)高血压患者的降压作用。方法:选取维持性血液透析的ESRD高血压患者18例,分二个阶段,先用含钙离子浓度1.5mmol/L(DCa1.5)透析液透析三个月,再用含钙离子浓度1.25mmol/L(DCa1.25)透析液透析三个月。观察透析开始(0h)、透析1小时(1h)、透析1个月及透析3个月结束时血压的变化;对比分析二种不同钙离子浓度透析各时段对血压的影响,同时观察对钙磷代谢的影响。结果:DCa1.5透析液透析收缩压(systolic bloodpressure,SBP)呈逐渐升高趋势,透析3个月SBP明显升高(P<0.05);而DCa1.25透析液透析SBP呈逐渐下降趋势,透析1个月时收缩压明显下降(P<0.05),3个月时SBP及舒张压(diastolic blood pressure,DBP)均明显下降,以SBP下降更为明显(P<0.01);而对血钙、血磷影响无统计学意义。结论:低钙(DCa1.25)透析可降低ESRD高血压患者的血压,对钙磷代谢无明显影响。Objective: To observe the two different concentrations of calcium dialysate on end - stage renal disease (ESRD) in patients with hypertension antihypertensive effect. Methods: Select the ESRD maintained hemodialysis patients with hypertension in 18 cases, sub -2 phases, first with the calcium ion concentration of 1. 5mmol / L (DCa1. 5) dialysate dialysis for three months, and then calcium ion concentration 1. 25mmol / L (DCal. 25) dialysate dialysis for three months. Results: DCal. 5 dialysate dialysis systolic blood pressure (systolic blood pressure, SBP) was gradually increased trend, dialysis 3 months SBP was significantly higher (P 〈 0. 05) ; while DCal. 25 dialysate dialysis SBP showed a gradual downward trend, dialysis 1 month when the systolic blood pressure decreased significantly (P 〈 0. 05 ), 3 months, SBP and diastolic blood pressure (diastolic pressure, DBP) decreased significantly to SBP decline was more marked (P〈0. 01 ) ; while serum calcium, blood Phosphorus was no significant impact. Conclusion: The low calcium (DCa1. 25 ) dialysis ESRD patients with hypertension can reduce blood pressure, had no significant effect on calcium and phosphorus metabolism.
