摘  要:目的:探讨老年人髋部骨折应用重建钉固定时,添加磷酸钙骨水泥强化股骨颈内2枚锁钉所产生的生物力学特性。方法:选择15个老年新鲜尸体的全长股骨干标本(年龄>55岁),随机分为3组,非强化组单纯重建钉固定,添加磷酸钙骨水泥(CPC)强化组,添加聚甲基丙烯酸甲酯(PMMA)强化组,固定后2h分别测定3组中股骨颈的2枚锁钉各自的初始松动加载力和最大轴向拔出力。结果:单纯重建钉固定时股骨颈内上方螺钉的初时松动加载力和最大轴向拔出力明显小于下方的螺钉;添加CPC和PMMA后均能够明显增加初时松动加载力和最大轴向拔出力,无统计学差别。结论:重建钉固定时股骨颈内上方螺钉的固定强度小于下方螺钉,更容易退钉。添加磷酸钙骨水泥能够大大提高重建钉的股骨颈内锁定螺钉的固定强度,与传统聚甲基丙烯酸甲酯骨水泥比较固定强度无明显差别。Objective:To evaluate biomechanical characteristics of calcium phosphate cement in the reinforcement of reconstruction intramedullay nail fixation for gerontal hip fracture.Methods:Fifteen samples of superior femurs were randomly divided into three groups :non-augmentation group;augmentation group with perfusing CPC;augmentation group with perfusing PMMA.The initial mobile reinforce and the maximum axial pull-out strength of three groups were measured for two hours after fixation.Results:Augmentation groups with perfusing PMMA and CPC prominently improved the initial mobile reinforce and the maximum axial pull-out strength than the non-augmentation group.There were no statistics significance between two augmentation groups.The initial mobile reinforce and the maximum axial pull-out strength of the above screw were less than the below screw.Conclusion:The reconstruction intramedullay nail fixation with perfusing CPC can improve the initial mobile reinforce and the maximum axial pull-out strength.The above locking screw of the neck of femur with reconstruction intramedullay nail fixation were more readily pulled out than the below locking screw.
