成纤维细胞生长因子-21与腹型肥胖的相关性研究

摘　　要：目的探讨成纤维细胞生长因子-21(fibroblast growth factor-21,FGF-21)与腹型肥胖及相关代谢指标的关系。方法纳入腹型肥胖61例,健康对照组113例,测量所有研究对象身高、体质量、腰围、血压;测定空腹状态下血糖、血脂、胰岛素、FGF-21等生化指标。结果①腹型肥胖组血浆FGF-21浓度(1.91±0.40)ng/mL高于健康对照组〔(1.75±0.45)ng/mL),P=0.020〕。②在校正年龄、性别后,血浆FGF-21水平与BMI(r=0.24,P=0.001)、腰围(r=0.16,P=0.033)、体脂含量(r=0.24,P=0.001)、甘油三酯(r=0.16,P=0.036)呈正相关。多元回归分析显示,血浆FGF-21水平与BMI、腰围、体脂含量、甘油三酯独立相关(P<0.05)。③校正年龄、性别因素后,FGF-21与腹型肥胖独立相关,其OR值为2.413(95.0%CI1.115～5.221,P=0.025)。结论 FGF-21可能是腹型肥胖的独立危险因子,可能是腹型肥胖发生发展的重要因素。Objective To investigate the association of plasma fibroblast growth factor-21（FGF-21） with abdominal obesity and other metabolic indicators.Methods Sixty one people with abdominal obesity and 113 healthy controls were recruited.The stature,avoirdupois,waist circumference and blood pressure of the participants were measured.Serum lipid,glucose,insulin and plasma FGF-21 levels of those participants were also determined.Results ①The participants with abdominal obesity had significantly higher FGF-21 levels than the healthy controls（（1.91±0.40） ng/mL vs.（1.75±0.45） ng/mL,P=0.020）.②Plasma FGF-21 levels were positively associated with BMI（r=0.24,P=0.001）,waist circumference（r=0.16,P=0.033）,body fat contents（r=0.24,P=0.001）,and triglycerides（r=0.16,P=0.036）after adjustment for age and sex.The multiple liner regression analysis identified BMI,waist circumference,body fat contents,and triglycerides as factors associated with FGF-21（P〈0.05）.③Plasma FGF-21 levels were found to be independently associated with abdominal obesity（OR=2.413,95%CI 1.115-5.221,P=0.025）.Conclusion FGF-21 is an independent risk factor for abdominal obesity.It is perhaps an important factor in the occurrence and development of abdominal obesity.
