摘 要:目的探讨血清同型半胱氨酸(Hcy)水平与2型糖尿病(T2DM)并发抑郁症的关系。方法 338例T2DM患者中,根据临床表现和汉密尔顿抑郁量表(HRSD)诊断抑郁症患者101例,根据HRSD评分评定其抑郁程度,分为轻、中、重三组。测定血清Hcy、FPG、HbA_1c。结果 T2DM患者Hcy水平显著高于对照组;糖尿病并发抑郁症患者血清Hcy水平显著高于非抑郁症组,抑郁症组有更高的HbA_1c(P<0.01),两组FPG水平无统计学差异。糖尿病并发抑郁症的轻、中、重三组比较,HRSD评分与Hcy、HbA_1c水平呈显著正相关。结论高Hcy血症可能是糖尿病患者抑郁症发病的重要危险因素。Objective To investigate the relationship between serum level of homocysteine (Hcy) and depression in patients with type 2 diabetes. Methods A total of 338 type 2 diabetic patients were enrolled into the study. Among them, 101 patients were diagnosed as depression according to clinical symptoms and scores of the Hamilton Rating Scale for Depression (HRSD). Comorbid depression was graded into mild, moderate, and severe stages based on the HRSD scores. The serum levels of Hcy,FPG,and HbA1 c were detected and analyzed among the different groups. Results Hcy level was significantly higher in depression plus diabetes group than in the control group [(16.09 ± 6.13)μmol/L versus( 11.36 ±± 3.20) μmol/L; P〈0.01]. Diabetic patients with depression bad significantly higher level of Hcy (P〈0. 01) and HbA1 c (P〈0. 01) than did the group with non- depression. But the FPG levels were similar between the two groups. Hcy level and the frequency of hyperhomocysteinemia were significantly higher in depression patients than in non-depression patients(all P〈0.01). The serum levels of Hcy and HbA1 c were significantly associated with the HRSD( Hcy: r= 0. 64, P〈 0. 05;HbA1 c r=0. 51,P〈0. 05). Conclusions Our results suggest that hyperhomocysteinemia may be a risk factor for depression in type 2 diabetic patients.
