摘  要:目的:探讨三维超声检查中孕期高危胎儿的临床价值。方法:对高危胎儿分系统进行二维超声检查,再行三维超声成像,明确胎儿情况,与引产或出生后的随访结果对照分析。结果:1667例高危胎儿进行三维超声检查,检出各种结构异常78例,漏诊8例,胎儿结构异常发生率5.16%。78例中严重的胎儿结构异常32例,占41.03%,轻微的胎儿结构异常46例,占58.97%。胎盘异常15例,发生率0.89%;羊水异常119例,发生率7.14%。1667例高危胎儿二维超声检查符合率70.93%,三维超声检查符合率87.21%。结论:该方法能对高危胎儿的结构异常进行明确诊断,是减少漏诊及误诊、降低医疗风险的重要方法。Objective : To explore the clinical value of three - dimensional ultrasound in examination of high risk fetuses in second trimester. Methods: All systems of the high risk fetuses were examined by two - dimensional ultrasound and three - dimensional imaging, then the results were compared to those after induced abortion or birth. Results: 1 667 high risk fetuses underwent three - dimensional ultra- sound, 78 fetuses were diagnosed as structural abnormality, 8 fetuses got missed diagnosis, the incidence of fetal structural abnormality was 5.16%. 32 fetuses were diagnosed as severe structural abnormality among 78 fetuses with structural abnormality, accounting for 41.03% ; 46 fetuses were diagnosed as mild structural abnormality among 78 fetuses with structural abnormality, accounting for 58.97%. The incidences of placental abnormality and amniotie fluid abnormality were 0. 89% ( 15 fetuses) and 7.14% ( 119 fetuses), respectively. The coincidence rates of two - dimensional ultrasound and three - dimensional ultrasound among 1 667 high risk fetuses were 70.93% and 87.21% , respec- tively. Conclusion : Three - dimensional ultrasound can diagnose structural abnormality of high risk fetuses definitely, which is a vital method to reduce misdiagnosis rate, omission diagnostic rate and medical risk.
