多囊卵巢综合征的中医证候与内分泌的相关性

摘　　要：目的探讨多囊卵巢综合征(PCOS)患者的中医证型与内分泌指标变化的相关性。方法采用前瞻性研究方法,收集广州市中医医院及广东省中医院2年的住院及门诊的多囊卵巢综合征的中医四诊信息及相关因素,建立数据库,采用数据库软件EPIdata3.2及SPSS13.0软件包对数据进行管理、处理。结果 PCOS中医证候类型主要表现为肾阳虚证、肝郁证、痰浊证、肾虚血瘀证和肾阴虚证。不同中医证型的体质量指数(BM I)、腰臀比(WHR)、空腹血糖、空腹胰岛素水平不同,有统计学意义。痰浊证的WHR值较肾阳虚证、肾阴虚证、肝郁证者高,肾虚血瘀证者较肝郁证者高;痰浊证的BM I、空腹胰岛素值较肾阳虚证、肾阴虚证、肝郁证、肾虚血瘀证者高;痰浊证的空腹血糖较肾阴虚证者高,均有统计学意义。PCOS中医辨证为痰浊证和肾虚血瘀证者多表现为中心性肥胖。结论 PCOS中医证型与内分泌指标呈现一定的相关性。Objective To study the correlation between Traditional Chinese Medicine syndrome and the changes of endocrine of patients with Polycystic ovary syndrome. Methods A prospective study methods was used. Data were collected from 138 cases of 2 hospital in 2 years, including in-patients and out-patients. The data base was established and statistical analysis was made with EPIdata 3.2 and SPSS 13.0. Results The traditional chinese medicine syndromes of PCOS are divided into deficiency of kidney-yang, liver depression, phlegm turbidity, deficiency of kidney and blood stasis and deficiency of kidney-yin. The clinical index, as BMI, WHR, FPG and Fins had significant differences among the five TCM syndromes of PCOS. The WHR of syndrome of phlegm turbidity is higher than that of deficiency of kidney-yang, deficiency of kidney-yin and liver depression, but in the deficiency of kidney and blood stasis, the WHR is higher than that of liver depression. The BMI and Fins of syndrome of phlegm turbidity is higher than that of deficiency of kidney-yang, deficiency of kidney-yin, liver depression and deficiency of kidney and blood stasis, FPG of syndrome of phlegm turbidity is higher than that of deficiency of kidney-yin. Central obesity were showed in syndromes of phlegm turbidity and kidney and blood stasis. Conclusion The TCM syndromes of PCOS were related to some endocrine factors and index to a certain extent.
