慢性肾衰竭中医证候分布与生存质量相关性研究

摘　　要：目的:通过对不同证候的慢性肾功能衰竭(CRF)患者生存质量状况的研究,探讨慢性肾衰患者中医证候与生存质量的关系及其规律。方法:采用自制的中医证候调查表及世界卫生组织生活质量简表横断面调查三地协作组198名CRF患者,根据CRF中医证候调查表及生存质量量表的相关内容建立数据库,研究两者的相关性。结果:慢性肾衰患者生存质量评分普遍较低,总评分为(58.33±9.65),其生存质量各维度受损程度按得分大小依次为生理领域、社会关系领域、环境领域和心理领域,生理领域评分最低为(50.45±12.17)。慢性肾衰肝肾阴虚患者生存质量得分最高(60.1±9.4),而阴阳两虚患者生存质量得分最低(45.9±2.3),其中肝肾阴虚、脾肾阳虚和阴阳两虚证型对社会领域、心理领域和生理领域的影响有统计学意义(P<0.05),其他证型对生存质量的影响无统计学意义(P>0.05)。结论:CRF患者的生存质量普遍受到影响。阴阳两虚、脾肾阳虚、肝肾阴虚对生存质量影响较大,在各证型中阴阳两虚型得分最低,而肝肾阴虚型得分最高,提示CRF病人的QOL可能与中医征候有关,但各证型相关程度不同。Objective：Throgh the research of different syndromes in chronic renal failure （ CRF） of quality of life,We study the relationship between quality of life and chronic renal failure syndromes of china medicine. Methods：A selfmade TCM questionnaire and the World Health Organization quality of life summary table the three cross -sectional survey of 198 cooperative group of CRF patients,according to CRF Syndrome Quality of Life questionnaire and the contents of a database related to study the relevance of those. Results：The quality of life score in patients with chronic renal failure is generally low,General Comments are divided into （ 58. 33 ± 9. 65） ,their quality of life scores according to the dimensions of the order of extent of the damage area for the physical,social relationships,and psychological areas of the field of environment,physiological score of the lowest （ 50. 45 ± 12. 17）. Patients with chronic renal failure,Yin defiaeny of liverkidney the highest quality of life scores （ 60. 1 ± 9. 4） ,while the dual deficiency of yin and yang quality of life in patients with the lowest score （ 45. 9 ± 2. 3） ,in which liver and kidney,spleen and kidney yang and deficiency of both yin and yang type of the the social field,the field of psychological and physiological impact of the field was significant （ P 0. 05） ,other syndromes on the quality of life was not significant （ P 0. 05）. Conclusion：CRF general quality of life of patients affected. the dual deficiency Yin and yang,deficiency of spleen and kidney,liver and kidney yin deficiency greater impact on quality of life,the yin and yang deficiency syndrome is the lowest,while the highest score,yin deficiency of liverkidney is suggesting that CRF patients with signs of QOL may be related to Traditional Chinese Medicine syndromes,but different syndromes related to different degrees.
