高频超声诊断肛周脓肿、肛瘘的临床价值

摘　　要：目的探讨高频超声诊断肛周脓肿、肛瘘的声像图表现及其临床应用价值。方法使用高频超声检查92例住院患者,所得结果与手术结果对照。结果肛周脓肿、肛瘘的声像图与周围正常组织存在明显差异;肛周脓肿单发18例,多发3例,诊断符合率100%;单纯性肛瘘34例,3例内口未明确显示,诊断符合率91%,复杂性肛瘘37例,8例内口或支瘘管漏诊,诊断符合率76%。结论高频超声检查对明确肛周脓肿的位置、数目、大小范围,肛瘘瘘管的走行、数目及内口的位置具有很高的诊断价值。Objective To investigate the ultrasonograhie manifestations of perireetal abscess and anal fistula and the application value of high-frequency in diagnosing in perirectal abscess andanal fistula. Methods A total of 92 patients were enrolled in this study, regular high-frequency were performed in these patients before operation. The accuracy of high-frequency ultrasonic was compared with a reference standard derived from surgical findings. Results The uhrasonograhic manifestations of perianorectal abscess and anal fistula were clearly different from around normal tissues. The coincidence rate of 18 single perianoreetal abscess and 3 complicated perianorectal abscess was 100% ,the coincidence rate of 34 single anal fistulas was 91% ,the coincidence rate of 37 complicated anal fistula was 76%. 3 single anal fistula patients and 8 complicated anal fistula patients were uncertain of the secondary trend of fistula or the location of fistula internal openings. Conclusion High-frequency ultrasonic can ascertain he location,number,size of perianorectal abscess and clearly estimate the trend and number of anal fistula and locate internal entrance. They were important in the diagnosis and treatment of perianorectal abscess and anal fistula.
