摘  要:为研究新生儿硬肿症酸碱紊乱的发生情况,对39例新生儿硬肿症患者进行血气分析和阴离子间隙(AG值)测定。结果示84.6%存在酸碱紊乱,其中81.8%为代谢性酸中毒,高AG代酸占70.4%;%重酸碱紊乱占23.1%,新生儿硬肿症酸碱紊乱及高AG代酸的发生率与有无合并症,体重高低无关(P>0.05),而与硬肿程度有关(P<0.01,P<0.05),但有合并症的硬肿症与单纯寒冷损伤其双重酸碱紊乱的发生率相差显著(P<0.05)。说明硬肿症患儿由于微循环障碍,全身主要脏器的代谢功能发生改变,是造成酸碱紊乱和高AG代酸的基础,而存在合并症将使酸碱紊乱变得更加错综复杂。AG值的测定对治疗有指导作用。建议硬肿症患儿应常规查血气和AG值,避免盲目补碱。To study the situation of acid-base dysequilibrium in neonatal sclerema, the AG value and blood gases analysis were determinated in 39 patients with neonatal sclerema. The re- sults showed that 33 cases (84. 6% ) had acid-base bysequilibrium, 27 (81. 8% ) of them were metabolic acidosis, The metabolic acidosis with high AG value hold 70. 4% and the double acid-base dysequilibrium hold 23.1%. The incidence of acid-base dysequilibrium and metabolic acidosis with high AG value did not related to complication and weight (P>0. 05), but connect with the extent of sclerema (P<0. 01, P<0. 05). The incidence of double acid- base dysequilibrium resulted from sclerema with complication was obviously different from that caused by simple cold injury (P<0. 05). It was concluded that the foundation of acid- base dysequilibrium and high AG value metabolic acidosis was the microcirculatory distur- bance and the changing of metabolic function of main organs. Acid-base dysequilibrium would become more complicated if with any complication. The determination of AG value was helpful for treatment. It was suggested that patient with sclerema ought to exmine the blood gases and AG value regularly, to avoid blindly supplement of base.
