农村社区人群经皮腰椎间盘髓核摘除术的临床疗效研究与干预

摘　　要：目的研究经皮腰椎间盘髓核摘除术(percutaneous lumbar diskectomy,PLD)的临床价值,筛选出一种适合农村社区人群的治疗方案。方法采用前瞻性临床研究法,以农村社区人群为目标,建立以农村社区为基础的术后康复干预计划,评价经皮腰椎间盘髓核摘除术治疗腰椎间盘突出症的临床疗效。结果26例共32个椎间盘均穿刺成功。经3～30个月的随访,24例症状完全消失,25例术前症状均明显缓解并能恢复原来的工作和生活,显效率为95.11%,椎间盘还纳1～4mm。结论经皮腰椎间盘髓核摘除术是安全有效经济易行的技术,疗效好见效快,所需治疗费用不高,农村社区人群容易接受,便于在农村社区广泛推广应用;术后康复干预能明显提高PLD中长期疗效。objective By means of studying the clinical value of percutaneous lumbar discectomy,we can select the suitability of treatment programs in rural communities. Methods Using a prospective clinical study method,and targeting rural communities’population,we can establish the rural community-based postoperative rehabilitation interventions. We also can evaluate the clinical efficacy of percutaneous lumbar discectomy for lumbar disc herniation. Results A total of 32 discs in 26 cases were successful puncture. After 3～30 months of follow-up,In 24 cases the symptoms disappeared completely. 25 cases of preoperative symptoms were significantly ease and be able to resume their original work and life. Remarkable efficiency of 95.11%. Intervertebral disc is sodium 1～4mm. Conclusion Percutaneous lumbar discectomy is not only safe,effective and economical,but also easy to operate. Base on the systemic therapy,the effect is also good and quick,the cost of treatment required is not high. People in rural communities are easy to accept. So that it will easy to be widely used in rural communities. Postoperative rehabilitation intervention can significantly improve long-term efficacy of PLD.
