复杂先天性心脏病合并下腔静脉畸形的彩色多普勒超声诊断

摘　　要：目的：探讨复杂先天性心脏病合并下腔静脉畸形的超声表现，以便作出正确诊断。方法与结果：经彩色多普勒诊断并经心血管造影或手术证实的５例下腔静脉畸形的患者，３例为下腔静脉肾上段中断由奇静脉连接，２例为下腔静脉畸形引流入左房。下腔静脉中断常合并肝静脉的畸形引流。同时，下腔静脉畸形与心房位置异常（心房对称）密切相关。结论：为了正确诊断下腔静脉畸形，应注意对剑下切面的观察，如果短轴切面见主动脉和下腔静脉对称结构消失，长轴切面能同时显示两条大血管，则高度怀疑下腔静脉畸形性。ABSTRACT Objective:To present 5 cases of anomalous inferior vena cava (IVC),with complicated congenital heart disease.Methods and Results:All the cases were diagnosed by CDFI and proved by cardioangiography or operation,3 without suprarenal inferior vena cava and azygos continuation of the abdominal cava,2 abnormal drainage of IVC to left atrium.All devoid suprarenal IVC patients had total anomalous hepatic venous connection to one or both atria via one or more veins.Anomaly of IVC closely associated with atrium isomerism.Conclusion:In order to provide detail information regarding IVC,particular attention should be paid to the short and long axis subcostal abdomen scans.Loss of AO and IVC symmetry with respect to the spine in short axis subcostal scan or two great vessels lying to the same side of spine in the long axis subcostal view suggests the possibility of IVC anomaly. KEY WORDS Anomalous inferior vena cava Color Doppler ultrasound 
