摘 要:目的研究彩色多普勒超声(color Doppler flow imaging,CDFI)监测四肢动脉损伤肢体远端侧支循环形成及其意义。方法回顾性分析77例临床高度怀疑四肢血管损伤行CDFI检查患者,重点观察肢体远端有无侧支循环,以及形成时间、部位、血流方向,脉冲多普勒(PW)测量血流动力学参数,包括流速(Vmax)、频谱形态、阻力指数(RI)。结果结果77例患者,正常17例,动脉损伤60例;动脉损伤患者中,34例肢体远端形成侧支循环,其中完全动脉胁迫征17例,完全断裂6例,动脉内膜挫伤并血栓形成1l例。肢体远端侧支循环血流动力学特征为双向、低速、小慢波或低阻类静脉样频谱。结论四肢动脉损伤肢体远端侧支循环具有特征性血流动力学改变,为CDFI定性诊断近端四肢血管损伤提供有力的证据。Objective To study the application of color Doppler flow imaging (CDFI) on collateral circulation of distal extremity with extremity arterial injury and its value. Methods A total of 77 cases highly suspected to have extremity arterial injury were examined with CDFI. Whether collateral circulation on distal extremity happened or not was focused on, including time and location of collateral circulation, direction of blood flow, as well as haemodynamics parameters measured with pulse Dopple (PW) including max velocity (Vmax), morphous of frequency spectrum and resistent index (RI) . Results In 77 cases, 17 cases were normal and 60 cases were with damaged artery. In these 60 cases, 34 cases were with collateral circulation in damaged artery, including 17 cases of complete entrapment syndrome, 6 cases of complete rupture and 11 cases of artery intima trauma and thrombosis. Haemodynamics charac- teristics of distal extremity collateral circulation were with two-way, slow velocity, slow wave and venous-like Dopple wave. Conclusions The haemodynamics of distal extremity arterial injury changes, which provides ample evidences for CDFI to diagnose extremity arterial injury.
