Tei指数及其相关参数定量评估子痫前期孕妇胎儿右室功能

摘　　要：目的探讨Tei指数及等容收缩指数(ICT/ET)、等容舒张指数(IRT/ET)等相关参数在评价止常胎儿和子痫前期孕妇胎儿心室功能中的临床价值。方法应用组织多普勒(tissue Doppler imaging,TDI)测量64例孕妇胎儿右室等容收缩时间(ICT),射血时间(ET)及等容舒张时间(IRT),获得右室Tei指数及其相关参数,其中正常孕妇30例(A组)、子痫前期轻度孕妇21例(B组)及子痫前期重度孕妇13例(C组)。并比较A、B和C三组间Tei指数及其相关参数。结果 C组胎儿右室Tei指数及IRT/ET明显高于A组,差异有统计学意义(分别为P<0.01与P%0.05);A、B两组间胎儿右室Tei指数及IRT/ET差异不明显,无统计学意义(P>0.05);A、B及C三组间的ICT、IRT、ET及ICT/ET差异均无统计学意义(P>0.05); 结论子痫前期重度不仅损害母体脏器,而且累及胎儿心室舒张功能;Tei指数及其相关参数是一种定量评价心室功能的多普勒指数,能简便、敏感、准确地评估胎儿心室功能。Objective To evaluate the clinical value of Tel index and related parameters including isovolumetrie contraction index （ICT/ET） and isovolumetric relaxation index （IRT/ET） in assessment of the cardiac function of normal fetuses and fetuses in pre-eciampsia. Methods Tissue Doppler imaging （TDI） was performed in a total of 64 fetuses. 30 normal fetuses of healthy mothers were assigned as the control group （Group A） while 21 fetuses of mildly pre-eclamptic mothers constituted Group B and 13 fetuses of severely pre eclamptic mothers constituted Group C. Tel index and related parameters were obtained by measuring the isovolumic contraction time （ICT）, ejection time （ET） and isovolumic relaxation time （IRT） of the right ventricle. Results Tel index and IRT/ET in group C were obviously higher than those in group A （P〈0.01 and P〈0.05, respectively）, but had no significant difference between group A and group B （P〉0.05） . There was also no significant difference between ICT, IRT, ET and ICT/ET among these groups （P〉0.05） . Conclusions Severe pre-eclampsia not only damages the organs of the pregnant women, but also endangers the ventricular function of the fetuses. Tel index and related parameters are simple, sensitive and reliable tools for quantitative assessment of fetal ventricular function.
