摘 要:目的:探讨不同临床类型慢性肝病与HBVBCP变异及IL-10、IL-12、TNF-α和IFN-γ关系。方法:以176例HBV感染慢性感染者(慢性乙型病毒性肝炎轻、中、重度,肝炎肝硬化,慢性重型肝炎和原发性肝癌)作为研究对象。采用PCR微板核酸杂交结合ELISA检测显示技术,对患者血清进行检测HBVBCP区核苷酸(nt)1762碱基A→T和1764G→A联合突变;采用双抗体夹心ELISA检测技术,检测IL-10、IL-12、TNF-α和IFN-γ水平。结果:BCP变异与不同临床类型肝病呈正相关(r=0.259,P=0.001)。IL-12、TNF-α和IFN-γ的水平除了慢性肝炎中度组外,其余各组与慢性肝炎轻度组间的比较有显著差异(P<0.05)。而IL-10水平比较在慢性肝炎轻度组与其他组间差异无统计学意义(P>0.05)。结论:BCP变异与慢性感染的不同临床类型呈正相关,随着病情的加重BCP变异的阳性率有随之增高趋势。IL-12、TNF-α和IFN-γ在HBV感染不同临床类型肝病中可能起到主要作用。Objective:To study the correlation between different clinical types of liver diseases and hepatitis B virus(HBV)basic core promoter(BCP)mutation and IL-10,IL-12,TNF-α,as well as IFN-γ in patients with chronic hepatitis B virus infection.Methods:One hundred and seventy-six patients [(chronic hepatitis B with mild,moderate and severe;liver cirrhosis,chronic fulminant and hepatocellular carcinoma(HCC)] with chronic hepatitis B virus infection were studied.The A to T mutation at nucleotide 1762 and G to A mutation at nucleotide 1764 were determined by the method of polymerase chain reaction(PCR)microplate hybridization ELISA.The serum cytokines(IL-10,IL-12,TNF-α and IFN-γ)were measured by specific-ELISA.Results:There was positively correlation between HBV BCP mutants and the progression of HBV-related liver diseases(r=0.259,P=0.001).The serum levels of IL-12,TNF-α as well as IFN-γ in group of chronic hepatitis B with severe,liver cirrhosis,chronic fulminant hepatitis and HCC were significantly higher than those in chronic hepatitis B with mild group(P〈0.05).But there was no significant difference in the level of IL-10(P〈0.05).Conclusion:There was positively correlation between HVB BCP mutants and the progression of HBV-related liver diseases.The positive rate of HBV BCP mutants was higher associated with the development of progressive liver diseases.The serum levels of cytokines(IL-12,TNF-α and IFN-γ)might have an positive effect on the development of progressive liver diseases.
