肿节风减轻鼻咽癌放射性口干的成本—效果分析

摘　　要：目的:探讨肿节风防治鼻咽癌(NPC)放射性口干的成本与效果。方法:选择初治Ⅰ、Ⅱ期NPC患者300例。按照患者知情同意分入单纯常规放疗(CR)组、常规放疗+肿节风(CRSG)组和单纯调强放疗(IMRT)组,每组各100例。采用成本-效果分析方法进行研究。结果:①成本:CR组[(24653.82±11862.23)元]和CRSG组[(24705.63±11982.19)元]成本均显著低于IMRT组[(55891.31±13315.62)元](均P<0.05),CR组与CRSG组成本比较差异无统计学意义(P>0.05);②效果:CRSG组(75.00%)和IMRT组(78.00%)I度口干的发生率均明显高于CR组(58.00%)(均P<0.05),CRSG与IMRT组I度口干的发生率比较差异无统计学意义(P>0.05);③成本-效果:C/E,IMRT组(716.56元)>CR组(425.07元)>CRSG组(329.41元)。C/△E(以CR组作参照),CRSG组(3.05元)低于IMRT组(1561.87元)。结论:肿节风对防治鼻咽癌放射性口干有效,费用低廉,具有临床经济学意义。Objective：To determine the cost-effectiveness of Sarcandra glabra on the prevention and treatment of radioactive xerostomia in patients with nasopharyngeal carcinoma（NPC）.Methods：Three hundred untreated NPC patients in stageⅠor Ⅱ were divided into conventional radiotherapy（CR）group,conventional radiotherapy plus Sarcandra glabra（CRSG）group,and intensity-modulated radiotherapy（IMRT）group;respectively.Each group comprised 100 patients.A cost-effectiveness analysis was also performed.Results：（1）Costs：The cost in CR group [（24 653.82±11 862.23）Yuan] and CRSG group [（24 705.63±11 982.19）Yuan] was significantly lower than that in IMRT group [（55 891.31±13 315.62）Yuan]（P〈0.05 for each）.There was no significant difference in the cost between the CR and CRSG groups（P〈0.05）.（2）Effects：The incident rate of radioactive xerostomia I° in CRSG（75.00%）and IMRT（78.00%）groups was higher than that in CR group（58.00%）（P〈0.05 for each）.There was no significant difference in the incident rate of radioactive xerostomia I° between the CRSG and IMRT groups（P〈0.05）.（3）Cost-effectiveness：The ratio of cost to effectiveness（C/E）in IMRT,CR and CRSG groups decreased gradually（716.56,425.07 and 329.41 Yuan;respectively）.C/△E（compared with CR group）in CRSG group（3.05 Yuan）was significant lower than that in IMRT group（1 561.87 Yuan）.Conclusion：Sarcandrae glabra can prevent and treat radioactive xerostomia in patients with nasopharyngeal carcinoma effectively.The cost of Sarcandrae glabra was cheap,and Sarcandrae glabra has active clinical economic significance.
