摘  要:目的颅内微出血是中枢神经系统常见的疾病,以临床症状作为主要的诊断依据。磁敏感加权成像的出现为颅内微出血诊断发挥重要作用。文中主要探讨磁共振磁敏感加权成像(susceptibility weighted imaging,SWI)技术以及在颅内微出血病变中的应用价值。方法收集2009年6月至9月经临床和手术病理确诊的颅内微出血患者45例:其中脑血管病20例、颅脑外伤13例、颅内肿瘤12例,分别行磁共振常规T1WI、T2WI、T2Flair、T1WI增强扫描以及磁敏感加权扫描。分析常规MR检查与磁敏感加权成像在显示病变微出血的差异。结果在45例微出血患者中,SWI序列均得到明确显示,表现为点状、条状、类圆形或环形的低信号影;在20例脑血管病变的患者中常规序列检出微出血的10例;颅脑外伤13例常规序列检出8例;颅内肿瘤12例常规序列检出7例。结论磁敏感加权成像可以充分的显示组织之间的磁敏感特性的差别,在诊断颅内微小出血病灶方面与常规序列相比有着明显的优势。Objective Cerebral micro-bleeding is a common problem in the central nervous system,whose diagnosis chiefly depends on clinical symptoms.This study aimed to evaluate susceptibility-weighted magnetic resonance imaging(SWI) in the diagnosis of cerebral micro-bleeding.Methods We collected 45 cases of cerebral micro-bleeding confirmed by clinical and surgical pathology,including 20 cases of cerebral vascular disease,13 cerebral trauma,and 12 intracranial tumor,all evaluated by conventional MRI T1WI,T2WI,T2Flair,T1WI enhanced scanning and susceptibility-weighted scan,and analyzed for the differences in the lesions shown by conventional and susceptibility-weighted MR imaging.Results SWI sequences clearly exhibited dot-,stripe-,round-or ring-shaped low signal shadows in the 45 patients.Conventional sequences detected 10 cases of micro-bleeding in the 20 patients with cerebral vascular disease,8 in the 13 patients with cerebral trauma,and 7 in the 12 patients with intracranial tumor.Conclusion Susceptibility-weighted imaging can adequately reveal the differences in the magnetic sensitivity between different tissues,and has distinct advantages over the conventional sequence in the diagnosis of intracranial lesions of micro-bleeding.
