摘 要:目的应用微电极阵列(MEA)研究钾通道阻断剂对快速起搏(RAP)兔右心耳场电位时限(fAPD)的变化。方法成年新西兰大白兔40只,体重2.5~3.0kg,雌雄不拘,由新疆医科大学动物实验中心提供,动物质量属于一级标准。随机分为3组,对照组(non.pace,n=8),起搏+钾通道阻断剂组(TEA、4-AP和BaCl2,每组n=8),起搏+胺碘酮组(n=8)。RAP24h后,迅速开胸取心脏,剪下右心耳,随即切片(厚度500um),将标本固定在MEA记录系统。分别记录正常对照组,给予阻断剂及胺碘酮组MEA形态和fAPD改变。结果对照组右心耳fAPD为(188.33±18.29)ms,起搏组fAPD为(173.91±6.83)ms。给予20mmol/LTEA阻断Ix,fAPD由(176.67±8.66)ms延长到(196.11±10.76)ms(P=0.012),5mmol/L4-AP阻断Ito,fAPD由(169.38±10.56)ms延长到(188.56±13.82)ms(P=0.005)。10~mol/L BaCl2阻断IKir,fAPD由(182.22±12.87)ms延长到(191.11±13.09)ms(P=0.039)。2×10-6mmol/L胺碘酮使fAPD由(167.38±13.67)ms延长到(185.00±15.14)ms(P=0.002)。结论应用MEA技术可真实客观反映心肌组织切片的电生理特性。24hRAP后,以阻断Ito,IKur IK1和IKs为主的钾通道阻断剂延长右心耳fAPD。胺碘酮可有效逆转或阻止右心耳fAPD的延长。Objective To explore the effects of potassium ion channel blocker on field action potential duration (fAPD) of right atrial appendage (RAA) during rapid atrial pacing (RAP) by Microelectrode arrays. Methods Fourty New Zealand white rabbits of either sex ( weight 2.5 to 3.0 kg, n = 40) were randomly divided into 3 groups:control groups, potassium ion channel blocker groups( TEA,4-Ap and BaCl2 )and amiodarone groups. All animals were anesthetized with pentobarbital (30 mg/kg followed by 5 mg/kg/h, i. v. ). The hearts were quickly removed and right atrial appendage (RAA) sliced (slice thickness 500 um). Each slice was perfused with Tyrode' s solution and continuously stimulated about 30 min. Control groups received Tyrode' s solution superfusion 10 min, blocker groups and amiodarone groups received blocker and amiodarone superfusion 10 min, respectively, fAPD of RAA and field action potential morphology was recorded with microeletrode arrays technique. Results In non-pace (control) groups, fAPD was ( 188. 33 ± 18. 29 ) ms after Tyrode' s solution superfusion, and fAPD was ( 173.91 ± 6. 83 ) ms after RAP. In pace/potassium ion channel groups,TEA and BaCl2 superfusion prolonged fAPD [ control vs. blocker : ( 176. 67 ± 8.66) ms vs. ( 196. 11 ± 10.76) ms, ( 182. 22 ± 12. 87 ) ms vs. ( 191.11 ± 13.09 ) ms with TEA and BaC12 superfusion, respectively, P 〈 0. 05].4-AP superfusion prolonged significantly fAPD[ control vs. 4-AP: ( 169. 38 ± 10. 56) ms vs. ( 188. 56 ±13.82)ms, P 〈 0.01 ] .In pace/amiodarone groups, 4-AP superfusion extended obviously fAPD [control vs. amiodarone:(167.38 ±13.67)ms (185.00 ±15.14) ms,P〈0.01]. Conclusions MEA was a sensitive and stable tissue action potential multiple-channel recording mapping system in animal heart slices, fAPD electrophysiologieal properties was similar to action potential duration (APD) and atrial effective refractory period (AERP).
