网状纤维染色对垂体疾病诊断及鉴别诊断的意义

摘　　要：目的探讨网状纤维染色在垂体疾病的诊断及鉴别诊断中的意义。方法收集大连大学附属新华医院2006年～2009年垂体外科手术切除标本22例,常规石蜡包埋,HE染色及网状纤维染色。结果22例病例中,17例术前诊断为垂体腺瘤,经网状纤维染色证实,15例为垂体腺瘤,1例为结节性垂体细胞增生,1例为淋巴细胞性垂体炎;3例囊性病变病理诊断为Rathke裂囊肿,其中1例经网状纤维染色证实伴有微腺瘤;另2例证实为垂体卒中,网状纤维染色勾画出了腺瘤的基本形态。结论应用网状纤维染色能很好地反映出病变中网状纤维的分布,在临床病理诊断中,对垂体疾病的诊断及鉴别诊断有重要意义。Objective To investigate the significance of reticulin staining in diagnosis and differential diagnosis of pituitary diseases.Methods 22 surgical speciments of pituitary lesion were collected from the patients during 2006 to 2009.H-E and reticulin staining were performed in paraffin-embedded tissue sections.Results Among 22 cases,17 cases were primarily diagnosed as pituitary adenomas before surgery.Approved by reticulin staining,15 cases were pituitary adenomas,and it was illustrated that there was scarcity of reticulin in pituitary adenomas studied by reticulin staining,one to be pituitary hyperplasia,accompanied with adrenal cortex hormones hypersecretory syndrome,and reticulin staining displayed expansion of aciniform configuration in this case;one to be lymphocytic adenohypophysitis with diffuse reticulin illustrated by reticulin staining;while the other 3 cystic lesions were diagnosed as Rathe cleft cyst,of which one was accompanied with microadenomas diagnosed though reticulin staining;and the last 2 cases were proved to be pituitary apoplexy.Conclusions The reticulin staining can reflect the distribution of reticulin rather well in the lesion.On pathologic diagnosis,it is of great significance in diagnosis and differential diagnosis of pituitary diseases.
