摘　　要：目的探讨肺Wegener肉芽肿的CT表现及其诊断价值。方法回顾性分析经手术切除(n=3)、肺穿刺活检(n=13)或支气管镜活检(n=2)确诊的18例肺Wegener肉芽肿患者的临床及CT资料。结果18例肺Wegener肉芽肿CT表现为肺内多发性结节和肿块15例,单发肿块2例,结节或肿块伴空洞13例,肺实变5例,肺梗死2例,支气管管壁增厚、管腔狭窄2例,纵隔淋巴结增大1例,胸膜增厚2例。结论两肺多发性结节、肿块并空洞,结节或肿块周围见"晕轮征"、"供养血管征"是肺We-gener肉芽肿的常见CT表现,病变的多发性、多样性、易变性和反复性对肺Wegener肉芽肿的诊断具有重要价值。Objective To observe CT manifestations and the value of CT in diagnosis of pulmonary Wegener granulomatosis.Methods Clinical data and chest CT findings of 18 patients with pulmonary Wegener granulomatosis confirmed pathologically（3 with surgical resection,13 with CT-guided transthoracic needle aspiration,2 with bronchoscopy） were retrospectively analyzed.Results Among 18 patients,chest CT showed pulmonary multiple nodules and masses in 15 patients,while single mass in 2 patients,nodules or masses with cavities in 13 patients,consolidation of lung in 5 patients,pulmonary infarction in 2 patients,thickening walls and narrowing lumens of bronchus in 2 patients,mediastinal lymph nodes enlargement in 1 patient and pleural thickening in 2 patients.Conclusion Pulmonary multiple nodules and masses with cavities,halo sign and feeding vessel sign around nodule or mass are common CT findings of pulmonary Wegener granulomatosis.The multiplicity,multiformity,inconstancy and repetition of the lesions are helpful for diagnosing pulmonary Wegener granulomatosis.
