摘  要:目的应用可视化仿组织体模聚丙烯酰胺凝胶,评价射频消融(RFA)与实时温度监控的相互影响。方法采用柠檬酸缓冲液配制聚丙烯酰胺凝胶,添加牛血清白蛋白作为温度指示剂,放置8通道尿道温度监控导管,制成带导管的仿组织体模。在距测温导管1 cm处插入电极针,于射频加温的同时观察射频对测温有无干扰以及测温误差的大小。以二维超声及常规MR T1W和T2W扫描检查射频消融灶,评价其形态、大小、消融体积及偏心比值。结果射频工作时测温曲线可见细小抖动,较停机测温时的曲线不平整、不光滑;从计算机测量结果来看,测温导管受射频仪干扰不明显。MRT1WI显示消融灶为高信号,与周围区域分界不清晰,而T2WI显示消融灶呈低信号,与周围高信号区域分界清晰。实时测温下形成的射频消融灶较无测温的消融灶偏小,测温时消融灶的中心略偏离射频针。结论实时测温对射频的影响不显著,基本能够满足临床热消融的测温要求。射频消融灶受实时测温的影响,表现为消融灶较细长,且圆心偏离射频针。Objective To evaluate the interaction of radiofrequency ablation(RFA) and realtime temperature monitoring with a transparent tissue-mimicking phantom mainly composed of polyacrylamide gel.Methods The phantom,which mainly contained polyacrylamide gel and bovine serum albumin as a temperature sensitive indicator,was prepared with citrate buffer.Before the gel's solidification,urethral catheter was inserted into the gel solution until the solution solidified.The electrod was inserted into the phantom vertically,1 cm away from the urethral catheter.The coagulation lesions were evaluated with 2D sonography,T1 and T2 weighted imaging of MR,including the shapes,sizes,volumes and decenter ratios.Results The temperature curves showed tiny trembles when radifrequency was operated,but differences were not significant.The results showed the coagulation regions during temperature monitoring were smaller than those without temperature monitoring.Conclusion The catheter can realtime monitor the temperature precisely,and has mild impact of RFA.Under realtime monitoring of temperature,the coagulation region of RFA become smaller and more eccentric than those without temperature monitoring.
