单核细胞趋化蛋白-1与白细胞介素-1β在煤工尘肺患者血清中的变化

摘　　要：目的探讨血清单核细胞趋化蛋白-1(MCP-1)和白细胞介素-1β(IL-1β)在煤工尘肺(CWP)中的变化及意义。方法采用酶联免疫吸附测定法检测188例CWP患者(CWP组)、64例与CWP组相同接尘条件的健康井下作业接尘矿工(接尘对照组)以及50例井上作业非接尘工人(正常对照组)血清中MCP-1和IL-1β的水平,并根据CWP期别、接尘年限及接尘性质等因素进行分析。结果接尘对照组血清MCP-1水平低于正常对照组(P<0.05);CWP组血清MCP-1水平显著高于接尘对照组(P<0.01),但仍低于正常对照组(P>0.05)。而3组血清IL-1β水平差异无统计学意义(P>0.05)。不同接尘性质CWP组间比较,矽肺组血清MCP-1水平显著高于煤肺组(P<0.05),而2组血清IL-1β水平则无明显差异(P>0.05)。不同期别、不同接尘年限CWP患者血清MCP-1和IL-1β水平差异均无统计学意义(P>0.05)。结论MCP-1可能在CWP炎症反应和肺纤维化形成过程中发挥作用。Objective To explore the changes of serum monocyte chemoattractant protein-1 （MCP-1） and interleukin-1β （IL-1β） in coal workers’ pneumoconiosis （CWP）.Methods The levels of serum MCP-1 and IL-1β were determined in 188 cases of CWP （CWP group）,64 cases of healthy miner （dust composed control group） and 50 cases of healthy control （control group） by enzyme-linked immunosorbent assay,and the changes of serum MCP-1 and IL-1β were further evaluated according to the stage of CWP,the duration of exposure to dust,and the character of the dust.Results Compared with the control,the levels of serum MCP-1 in dust composed control group were significantly lower （P0.05）,the levels of serum MCP-1 in CWP were significantly higher than that of dust composed control（P0.01）.However,no significant differences were found in the levels of serum IL-1β among 3 groups,and in the serum levels of MCP-1 and IL-1β in CWP according to stage and the duration of exposure to dust.Compared with the coal pneumoconiosis subgroup,serum level of MCP-1 in silicosis subgroup was significantly higher （P0.05）,while the level of serum IL-1β was statistically unchanged between coal pneumoconiosis and silicosis.Conclusion The changes of serum MCP-1 could concomitantly participate in the occurrence and development of CWP.
