摘  要:目的检测妊娠期糖尿病(gestational diabetes mellitus,GDM)患者脂肪组织磷脂酰肌醇-3激酶(phosphatidylinositol 3-kinase,PI-3K)活性的改变,探讨GDM产生胰岛素抵抗(insulin resistance,IR)的分子机制。方法采用放射免疫法测定35例正常妊娠妇女及35例GDM患者血清空腹胰岛素(fasting insu-lin,FIN)的浓度,葡萄糖氧化酶法测定血浆空腹血糖(fasting plasma glucose,FPG);免疫沉淀、薄层层析及γ液闪计数方法检测PI-3激酶的活性,并进行分析比较。结果 GDM组患者FPG[(7.85±3.51)mmol/L]、FIN[(25.17±3.76)mU/L]及HOMA-IR(1.75±0.12)均显著高于对照组,差异有统计学意义(P均<0.01);与对照组比较,GDM组PI-3K活性(85.75%)下降(P<0.01);GDM组PI-3K与孕晚期体重、孕晚期体重指数及HOMA-IR呈负相关(r=-0.59、r=-0.63、r=-0.74,P均<0.01)。结论 GDM患者PI-3K参与了胰岛素信号通路中信号分子的生物学作用,导致IR的形成。Objective To detect the phosphatidylinositol 3-kinase activity in adipose tissue of diabetes mellitus(GDM) patients,thus to explore the molecular mechanisms of insulin resistance of GDM.Methods Samples from patients with GDM(GDM group,n=35),and healthy pregnant women(controls group,n=35) were collected.Fasting insulin(FIN) was measured by radioimmunoassay.Fasting plasma glucose(FPG) was determined by oxidase assay.PI-3 kinase activity was detected by immunoprecipitation,thin-layer chromatography and gamma scintillation counting.The results were analyzed statistically.Results The levels of serum FPG[(7.85±3.51) mmol/L],FIN [(25.17±3.76) mU/L] and HOMA-IR(1.75±0.12) in GDM group were significantly higher than those in control group(P〈0.01);PI-3 kinase activity(85.75%) was significantly decreased in GDM group compared with control group(P〈0.01)and negatively correlated with HOMA-IR(r=-0.74,P〈0.01).Conclusions PI-3 kinase may affect the insulin signaling pathway,and takes part in the insulin resistance of gestational diabetes mellitus.
