成人房间隔缺损并肺动脉高压外科治疗13例报道

摘　　要：目的观察外科治疗成人房间隔缺损并肺动脉高压的疗效。方法回顾性分析2004年1月至2009年6月13例房间隔缺损并肺动脉高压的患者手术资料。结果术后心律、血压均平稳,呼吸辅助通气12～24h,均痊愈出院,随访(32.67±7.63)个月,活动正常,口唇发绀及下肢水肿消失。心功能Ⅰ级5例,Ⅱ级8例。X线胸片示心影缩小,心胸比率降至(0.55±0.12)。超声心动图右心房及肺动脉内径明显缩小,肺动脉收缩压降至(21.42±4.39)mmHg。结论房间隔缺损并肺动脉高压患者效果满意,应尽早手术治疗,围术期应合理地处理以降低手术死亡率和并发症的发生。Objective To observe the effect of surgery on adult atrial septal defect and pulmonary hypertension. Methods A retrospective analysis was made of surgical data 13 cases of atrial septal defect and pulmonary hypertension from Jan. 2004 to June 2009. Results The heart rate and blood pressure in 13 cases were stable ,breathing assisted ventilation and 12 -24h,and all were discharged,with（32.67 ± 7.63） months of follow-up,normal activities,disappeared lips cyanosis and lower extremity edema. Five cases showed grade I cardiac function and 8 cases,grade II cardiac function. X-ray film showed reduced heart, and the cardiothoracic ratio was decreased（0.55 ± 0.12）. Echocardiograms showed that the right atrium and the pulmonary artery diameter were significantly reduced and the pulmonary artery systolic pressure reduced to （21.42 ± 4.39）mmHg. Conclusion The surgical treatment of atrial septal defect and pulmonary hypertension as soon as possible shows satisfactory results and the perioperative treatment should be reasonable in order to reduce operative mortality and complications.
