摘 要:目的:评价血涂片复检、复查对初步纠正电阻抗法(PLT-I)引起的血小板假性降低的意义。方法:选取64例PLT-I计数血小板提示有血小板直方图异常的标本,分别用光学法(PLT-O)计数血小板和涂片镜检观察血小板数量和形态分布并间接计数血小板的数量,以PLT-O法计数血小板为参照,评价涂片间接计数法和PLT-I法计数血小板的准确性。结果:在64例标本中,1例患者存在小红细胞和红细胞碎片,14例患者同时存在小红细胞和大血小板,其余49例存在比例不等的大血小板,其中5例患者存在直径>6μm的巨大血小板。对于存在大血小板的49例标本,PLT-I法与PLT-O法计数血小板相比,t=3.99,P<0.01,2组数据差异有统计学意义;显微镜间接计数与PLT-O法的相比,t=0.088,P>0.05,2组数据差异无统计学意义。结论:涂片镜检复查对发现血小板减少及假性降低有提示作用,且显微镜间接计数对初步纠正由大血小板引起的血小板假性降低有明显意义。Objective:To evaluate the significance of the blood smear re-examination on platelet pseudo-reduction counting by impedance.Method:64 cases of patients were selected by XT-1800i,platelet histogram of these samples were presented to be abnormal by PLT-I.PLT of the these samples were tested again by PLT-O and the morphology and the number of PLT by the blood smear were observed within 4 hours respectively.The accuracy of indirect PLT counting by blood smear and PLT-I were compared,using the PLT-O as a reference method.Result:Among these 64 patients,1 had small RBCs,14 had small RBCs and large platelets,the remaining 49 patients had large platelets in which 5 had grand PLT with diameter6μm.The result of PLT-I had a significant difference to that of PLT-O (t=3.99,P0.01),and there was no difference between the indirect PLT counting by blood smear and PLT-O(t=0.088,P0.05).Conclusion:Blood smear re-examination would help to discover the PLT reduction and pseudo-reduction tested by PLT-I,and indirect PLT counting by blood smear could preliminarily correct the platelet pseudo-reduction counted by PLT-I caused by large platelets.
