摘  要:目的明确从AIDS患者血液、骨髓中检出的病原体为马尔尼菲青霉菌。方法取患者血液、骨髓于血培养瓶中置Bacter/Alert3D血培养仪中培养,待血培养仪报阳性即作革兰染色,并转种至马铃薯斜面培养基于27℃和37℃孵育,观察真菌生长情况及菌落形态,并作乳酸棉兰染色于显微镜下观察菌体特征。结果该菌为一双相真菌,27℃见菌丝相,呈淡黄色绒毛样,并产生酒红色色素,镜下可见典型的帚状枝及孢子链;37℃见酵母相,不产生特征性色素,镜下见酵母样细胞。结论从体内分离出马尔尼菲青霉菌,即可诊断为马尔青霉菌病,也可排除组织胞浆菌感染。OBJECTIVE To detect the Penicillium marneffei (PM) from the blood and bone marrow in a patient with AIDS. METHODS The blood or bone marrow was inoculated in the culture flask and inseted the hemoeulture meter of Baeter/Alert 3D. If it was positive then to make Gram staining,and inoculate on poteto dextrose agar at 25 ℃ and 37 ℃, respectively. The growth and the fungal colony of PM were inspected and the characteristics of PM by stain of lacto-phenol cotton blue were observed under microscope. RESULTS The fungus was a dimorphic, it appeared to be a filamentous type colony with light yellow villi,and produced wine color pigment in agar, and present typical bloom-like twigs and spore-chains under microscope. It appeared to be yeast phase and without characteristic pigment. It showed yeast-like cells under microscope. CONCLUSIONS The diagnositic standard of PSM depends on isolation of PM from patients, with exclusion of Histoplasma infection.
