出院后随访对普外科手术部位感染率的影响

摘　　要：目的了解普通外科手术部位感染(SSI)实际发生情况,探讨出院后随访对手术部位感染率的影响。方法对某三级甲等教学医院2008年3月1日-9月30日普通外科1589例次住院手术患者采用前瞻性队列研究的方法 ,对其医院感染发生情况进行监测;并对手术患者建立出院后追踪档案,出院30 d时电话随访患者切口愈合情况。结果 1589例次手术患者中发生SSI 155例次,例次感染率为9.75%,其中住院期间发生99例次(63.87%),出院后发生56例次(36.13%);出院患者实际回访率达95.78%,随访感染主要发生在Ⅰ、Ⅱ类手术切口类型,占随访感染的87.50%,出院后56例次SSI主要发生在乳腺、阑尾、胆道手术,占随访感染的51.79%。结论在进行SSI目标性监测研究时,应使用标准的监控方法 ,加强对手术患者出院后的追踪随访,才能获得更有效的监测数据,有助于获得SSI的真实情况。OBJECTIVE To study objective supervision of nosocomial infection in the general surgery patients, and assess the influene of postdischarge infection surveillance on surgical site infection rates for general surgery procedures. METHODS 1589 hospital patients of general surgery were selected from a Grade Ⅲ Class A teaching hospital from March to September, 2008 for a prospective cohort study, and the nosocomial infection was supervised. The follow-up records of operation patients were established after they were discharged from hospital, and these patients were followed-up 30 days about their wounds. RESULTS Among the 1589 hospital patients, the surgical sites of 155 eases were infected and the infection rate was 9.75 %. Surgery infection occurred to 99 cases during hospitalization （63.87%） and to 56 cases after they were discharged from hospital （36.13%）. The rate of follow-up was 95.78%. the major postdischarge infections were type one and type two operation infections, which accounted for 87.50% of all. The most of 56 postdischarge infections cases were breast surgery, appendix surgery and biliary surgery , which accounted for 51. 79%. CONCLUSIONS When we study on objective supervision of surgical-site infection, we should use standard supervision methods, increasing the follow-ups of outpatients can help us understand the true information of SSI.
