妇科手术患者围手术期抗菌药物使用的量化管理

摘　　要：目的规范执行妇科清洁手术常规不应用抗菌药物与清洁-污染手术围手术期预防用药方案及选用推荐用药。方法选择2008年1月-2009年6月该院所有妇科清洁手术病例及4、10月份清洁-污染手术病例作为研究对象,根据卫生部《抗菌药物临床应用指导原则》制定量化管考核方案,组织培训、指导,落实随访、考核,定期统计、分析、反馈、执行并督导整改。结果 1291例妇科清洁手术病例中未应用抗菌药物的为70.33%,297例清洁-污染手术病例中执行围手术期预防用药方案的为45.12%,680例使用抗菌药物病例中选用推荐用药的为62.21%,而手术部位感染率清洁手术为0.77‰,清洁-污染手术为1.68%。结论对妇科清洁手术不常规使用抗菌药物,清洁-污染手术执行围手术期预防用药方案以及围手术期选用推荐用药等进行量化管理是必需的,也是行之有效的。OBJECTIVE To investigate whether clean gynecologic operation was done without routine antibacterial usage and whether recommended antibacterial was used in type Ⅱ incision. METHODS All clean gynecologic operation and some type Ⅱ incision cases during a certain period of our hospital were studied. We instituted a quantized management project, including training, guiding, follow up, checking, regular statistics, analyzing, feedback, supervising and directing. RESULTS Among 1291 patients, 70.33% clean gynecologic operations were done without antibacterial usage, and 45.12 % of type Ⅱ incision cases had prophylactic antibacterial applications. The rate of recommended antibacterial usage was 62.21%. The infection rate after clean operation was 0.77% and 1.68 % in type Ⅱ incision. CONCLUSIONS The quantized management of perioperative antibacterial usage for gynecology is effective and necessary.
