摘  要:目的:研究冠心病心绞痛痰浊痹阻证(痰浊证)、心血瘀阻证(血瘀证)患者的血浆代谢组学特征,探讨代谢组学方法在研究冠心病心绞痛中医证型中的应用。方法:对10例痰浊证患者和10例血瘀证患者的血浆样本进行氢核磁共振(1HNMR)检测。用主成分分析(PCA)与偏最小二乘判别分析(PLS-DA)处理图谱数据,研究2组血浆代谢物图谱的差异。结果:1HNMR图谱模式识别分析显示,2组血浆样本代谢物差异显著。痰浊证组乳酸含量较高,血瘀证组丙氨酸含量较高,说明2组都存在能量代谢紊乱;痰浊证组低密度脂蛋白、极低密度脂蛋白、脂类化合物、酮体含量均高于血瘀证组,高密度脂蛋白、不饱和脂肪酸含量均低于血瘀证组,说明痰浊证组比血瘀证组存在更严重的脂代谢紊乱;痰浊证组葡萄糖、半乳糖、N-乙酰糖蛋白含量均高于血瘀证组,说明痰浊证组比血瘀证组存在更严重的糖代谢紊乱;痰浊证组天门冬氨酸、异亮氨酸、谷氨酞胺、白蛋白赖氨酰含量均高于血瘀证组,丙氨酸、瓜氨酸、精氨酸、脯氨酸含量均低于血瘀证组,说明2组氨基酸代谢紊乱程度相当。结论:2组血浆样本的1HNMR代谢物图谱存在差异,能从代谢组学分析中找出特异的标志性代谢物。血浆代谢物变化能在一定程度上区分冠心病心绞痛的不同中医证型。Objective:To study the metabonomics characteristics of angina pectoris (AP)induced by coronary artery disease (CAD) with phlegm-turbidity syndrome and blood-stasis syndrome, and to explore the metabonomics indexes for the classification of syndrome patterns of CAD-induced AP. Methods: Nuclear magnetic resonance (1H NMR)was used to analyze the plasma samples from 10 phlegm-turbidity CAD-induced AP patients and 10 blood-stasis CAD-induced AP patients. Principal component analysis (PCA) and partial least squares discriminatory analysis (PLS-DA) were used to analyze the spectrogram data, and the differences of spectrogram data of plasma metabolites between the two groups were studied. Results: The results of 1H NMR showed that the differences of metabolin in the plasma samples from the two groups were significant. Lactic acid content was higher in phlegm-turbidity group, and lactamine content was higher in blood-stasis group, indicating the disorder of energy metabolism. In phlegm-turbidity group, the contents of low density lipoprotein, very low density lipoprotein, lipoid substance and ketone body were higher, and the contents of high density lipoprotein and unsaturated fatty acid were lower than those in blood-stasis group, indicating the severe disorder of lipid metabolism in phlegm-turbidity group. The contents of glucose, galactose, and N-acetyl glycoprotein were higher in phlegm-turbidity group than those in blood-stasis group, indicating the severer disorder of glucose metabolism in phlegm-turbidity group. The contents of aspartic acid, isoleucinein, glutamyl phthalimide and lysyl albumin were higher, and the contents of lanine, citrulline,arginine and proline were lower in phlegm-turbidity group than those in blood-stasis group, indicating the disorder of amino acid metabolism in the two groups. Conclusion: There exists 1H NMR spectra difference of metabolin in the plasma samples from the two groups, and the specific metabolic product can be found according to the metabonomics characteristics.
