孟鲁司特对婴幼儿毛细支气管炎预后的影响

摘　　要：目的:观察孟鲁司特对婴幼儿毛细支气管炎疗效及喘息复发的影响。方法:将2008年10月～2009年4月在我院住院治疗的60例毛细支气管炎的患儿随机分为观察组和对照组各30例,两组患儿均采用相同的综合治疗方法,观察组在综合治疗的基础上加用孟鲁司特,每晚睡前口服,在症状发作1周内服用,持续4周后停药。治疗前后分别检测LTC4,ECP。结果:观察组症状、体症消失时间较对照组明显缩短(P<0.01),6个月内喘息复发率明显低于对照组(P<0.05)。观察组和对照组出院后随访1个月,血清LTC4,ECP均有下降,而观察组在服药4周后与对照组相比较,血清LTC4,ECP下降更加明显,差异有统计学意义,且LTC4下降有非常显著差异(P<0.01)。结论:口服孟鲁司特治疗婴幼儿毛细支气管炎,可以改善症状,缩短病程。连续服用4周,可降低喘息再发频率,改善预后。Objective： To observe the influence of the curative effect of montelukast on infants with bronchiolitis and recurrence rate of gasp. Method： Sixty infants with bronchiolitis were randomly divided into two groups ： the observation group and the control group, both were 30 cases respectively. All of the patients were treated with identical combined treatment;on the basis of combined treatment, the observation group were used montelukast, taking once daily orally, with a treatment course of 4 weeks. Before starting treatment and 4 weeks later, concentration of serum leukotriene C4 and eosinophilic cationic protein were measured. Result： The time of the observation group＇ s symptoms and signs returned to normal state was obviously earlier than that of the control （ P 〈 0. 01 ） , and the recurrence rate of gasp within 6 months obviously lower in the observation group than that of the control. The serum LTCA and ECP of all of the patients that were followed up for 4 week after discharge were decreased,which was more obviously in the observation group. There was significant difference in the serum LTC4 and ECP of the observation group and the control after 4 weeks. Serum LTCA discharge were decreased （P 〈0. 01 ） . Conclusion： Montelukast can take an effect of improving symptoms and reducing course of treatment on infants with bronchiolitis. Taking montelukast continuously for 4 weeks can lower recurrence rate of gasp and improve prognosis.
