药物凝胶局部介入治疗对糖尿病并菌阳空洞肺结核的临床观察

摘　　要：目的:探讨经纤支镜结核空洞局部灌注含药凝胶治疗糖尿病并菌阳空洞肺结核的临床疗效。方法:将210例糖尿病并菌阳空洞肺结核患者纳入观察范围,随机分成研究组(简称A组)108例及对照组(简称B组)102例。A组经纤维支气管镜灌注含药凝胶和用抗结核药物治疗;B组仅用抗结核药物治疗。结果:A组在治疗1、3、6个月后痰菌转阴率、结核病灶治疗有效率、结核空洞治疗有效率及血糖检测正常率均显著高于B组;两组并发症及不良反应发生率差异无统计学意义;随访2年时A组及B组的痰菌复阳率和血糖检测正常率具有统计学差别。结论:药物凝胶局部介入治疗对糖尿病并菌阳空洞肺结核的治疗安全有效,近、远期疗效确切,痰菌复阳率低,血糖控制理想。Objective：To evaluate the clinical outcome and safety of the treatment of diabetic tuberculosis patients presented with bacteriological positive and cavitary in pulmonary with drug gel in the intervention treatment by fiberbronchoscope.Methods：210 diabetic tuberculosis patients presented with bacteriological positive and cavitary in pulmonary were included in the study. They were divided into experiment group （for short group A） and control group （for short group B） by random：108 cases in group A and 102 cases in group B.The group A were treated with containing drug gel by bronchofiberscope and medication,the group B were treated only with medication.Results： Sputum negative conversion rate,focimarked absorption rate,cavity curative effect rate and blood sugar normality rate at the end 1,3 and 6 month therapy respectively were higher than those in group B.Complication and adverse reaction of patients in the two groups had no significant difference.At the final follow up after 2 years,the sputum positive conversion rate and blood sugar normality rate of group A and group B patients had significant difference.Conclusions：Intervention therapy is a effective safe treatment for daibetic tuberculosis patients presented with bacteriological positive and cavitary in pulmonary in long-term and short-term.This treatment plays a important role in reducing long-term replapse rate and is effective for controling blood sugar.
