摘  要:目的:探讨三维超声体积自动测量技术测量小脑体积的可行性,并建立不同孕周胎儿小脑体积正常范围参考值。方法:选取研究对象为245例正常胎儿,胎龄20~36周,进行小脑三维超声容积取样,启动体积自动测量技术应用程序,自动分析出体积测量值,分析其与孕周、双顶径的相关性。并随机选取30例行三维测量的组内及组间重复性检验。结果:三维超声测量小脑体积的重复性检验各组间差异无统计学意义(P>0.05)。胎儿小脑体积随孕周增加而逐渐增加(r=0.950,P<0.001),并与双顶径增长一致(r=0.963,P<0.001)。结论:三维超声容积测量功能检测胎儿小脑体积是可行的。所获数据为产前判断小脑发育不良和相关异常,准确推算孕周提供参考。Objective: Using virtual organ computer aided analysis (vocalTM) of three-dimensional ultrasound(3D-US), fetal cerebellum volume (FCV) was measured in order to investigate the feasibility of this method and establish normal growth chart. Methods: 245 healthy fetuses from the 20th week to 36th week were scanned by 3D-US in this study. FCV was measured by the program of vocalTM. The correlations of the cerebellar volume to the pregnant week and BPD were examined with the scatter graph and polynomial regression analysis. The intraobserver and interobserver reproducibility of 3D-US was compared on the assessment of FCV.Results:There was no significant diference between the intraobserver and interobserver reproducibility of 3D-US on the assessment of FCV (P〉0.05).The cerebellum grew in a curve fashion from the 20th week to the 36th week and the growth pattern was correlated with gestational age(r=0.950,P 〈 0.001) and BPD(r=0.963, P 〈 0.001). Conclusions:Accurate volume measurement with 3D-US ultrasound may be applied to evaluate cerebellar development and allow quantitative evaluation.The reproducibility of 3D-US on the assessment of FCV is good.Knowledge of normal cerebellar appearance may help identify developmental anomalies and enable accurate prenatal counseling.
