子癎前期型肾病综合征的临床特点及围产结局分析

摘　　要：目的:分析子癎前期型肾病综合征患者的临床特点及其围产结局。方法:巢式对照分析我院2005年1月—2009年1月23例合并肾病综合征(肾病组)和67例未合并肾病综合征(对照组)的重度子癎前期患者的临床资料,比较2组血压、生化指标和母婴并发症发生情况。结果:肾病组平均年龄小于对照组,初产妇比例高于对照组,终止妊娠前平均住院时间短于对照组(均P<0.05)。肾病组最高收缩压低于对照组,血清总蛋白及白蛋白均低于对照组,24h尿蛋白定量、尿素氮及血肌酐均高于对照组(均P<0.05)。肾病组子癎、大量腹水、胎儿宫内窘迫、宫内生长受限及围产儿死亡的发生率高于对照组(均P<0.05)。结论:子癎前期型肾病综合征患者年龄小、病情重、进展快,母婴预后差,早诊断、适时终止妊娠可提高围产质量。Objective：To investigate the clinical characteristic and outcome following preeclampsia with nephritic syndrome pregnancy（NSP）.Methods： The clinical data of 23 cases with NSP（NSP group） and 67 cases with other preeclampsia （control group） were analyzed by nested case-control study in this hospital during January 2005 to January 2009.The values of blood pressure,biochemical indicators,maternal and fetal complications were compared between the two groups.Results： The mean age of NSP group was significantly younger than that of control group （P 0.05）.There was a higher rate of primipara in NSP group compared with that of control group （P 0.05）.There was a shorter average hospital those stay in NSP group than that in control group （P 0.05）.The systolic blood pressure was lower in NSP group than that in control group.The total serum protein and albumin were lower in NSP group than those in control group.The 24-hour proteinuria,urea nitrogen （BUN） and creatinine（Cr） were higher in NSP group than that in control group respectively（P 0.05）.The incidence of eclampsia,ascites,fetal distress,fetal growth restriction and prenatal death were significantly higher in NSP group than that in control group （P 0.05）.Conclusion： Perinatal prognosis of NSP is poor.The early diagnosis and pregnancy termination in time may improve perinatal quality.
