摘　　要：目的：探讨三皮质螺钉固定治疗合并下胫排联合韧带损伤的踝关节骨折脱位的疗效。方法：自2005年3月～2008年7月，手术治疗合并下胫排联合分离的踝关节骨折脱位患者28例，根据Weber—AO分型：B型19例，C型9例。钢板固定外踝，螺钉固定骨折的内踝和后踝。复位后仍不稳定的下胫腓联合，用螺钉固定，均穿过三层皮质。结果：所有患者获得12～36个月（平均18．5个月）随访，根据AoFAs评分：优20例，良5例，可或差3例，优良率为89．3％。结论：通过三层皮质螺钉固定可以达到下胫腓关节的动力固定，操作简便，疗效可靠。Objective： Explore the three cortical screw fixation oftibial row of the merger under the ankle joint ligament injury in the efficacy of fracture-dislocation. Method： Since March 2005 -July 2008, Surgical treatment of tibial row of the merger under the separation of the ankle joint fracture and dislocation of 28 patients. According to Weber-AO classification： B-type in 19 cases, C-type in 9 cases. Fixation of lateral malleolus, screw fixation of ankle fractures and post-ankle. Using screws through the three-cortex fixation tibiofibular joint, if the reset after the remains precarious. Result： All patients received 12 to 36 months （mean 18.5 months） follow-up, Excellent in 20 cases, good in 5 cases, may or poor in 3 cases, the fine rate was 89.3%. Conclusion： Through the three-cortex screw fixation can be achieved dynamic fixed tibiofibular joint fixation. The method is simple, effective and reliable.
