胰岛素释放试验在心脑血管病诊疗中的应用研究

摘　　要：目的探讨胰岛素释放试验(IRT)结果与高血压(HT)、冠心病(CHD)和脑梗死(CI)等心脑血管病的相关性,研究IRT指标在心脑血管病诊断、治疗监测、预后判断中的价值。方法葡萄糖氧化酶法(GOD)检测空腹血糖(FBG)和糖耐量(OGTT);放射免疫分析法(RIA)检测胰岛素(INS)和C-肽(C-P),计算胰岛素敏感指数(ISI)。比较疾病组与对照组各指标结果间的差异。结果 4组受检者FBG水平、空腹INS水平比较,差异均有统计学意义(P<0.01);餐后1hOGTT、INS水平比较,差异均无统计学意义(P>0.05);餐后2h与3hOGTT、INS水平比较,差异均有统计学意义(P<0.05)。4组受检者ISI、C-P浓度比较,差异均有统计学意义(P<0.05)。结论高血压、冠心病和脑梗死患者均伴有不同程度的高胰岛素血症(HIS)和胰岛素抵抗(IR)。IRT可提示高血压、冠心病和脑梗死的危险程度,可作为诊疗和监测高血压、冠心病和脑梗死不可缺少的指标,应引起临床的重视。Objective To study the correlation of insulin release tests （IRT） results with hypertension （HT）,coronary heart disease （CHD） and cerebral infarction （CI） so as to reveal the value of IRT results in diagnosis,treatment and prognosis of cardiocerebrovascular diseases.Methods Fasting blood glucose （FBS） and oral glucose tolerance test （OGTT） were determined by glucose oxidation （GOD）;insulin （INS） and c-peptide （C-P） were determined by radioimmunity assay （RIA） and insulin sensitivity index （ISI） was calculated.The differences in these indexes between the disease groups with normal group.Results The differences in FBG and fasting INS levels among the four groups were all statistically significant （P0.01）.The differences in OGTT and INS levels one hour after the meal showed no statistical significance （P0.05）,but the differences two and three hours after the meal were statistically significant （P0.05）.The differences in ISI and C-P results among the four groups were all statistically significant （P0.01）.Conclusion The patients with HT,CHD and CI have hyper-insulinemia（HIS） and insulin resistance （IR）to different degrees.IRT may give a clue to the risk degrees of HT,CHD and CI,and it should be clinically paid more attention to and applied as an indispensible laboratory index about treating and monitoring HT,CHD and CI.
