摘  要:目的了解鲍曼不动杆菌医院感染现状及相关危险因素;探讨鲍曼不动杆菌的耐药性和Ⅰ型整合子的关系。方法对2007年我院由鲍曼不动杆菌导致的医院感染89例患者的危险因素进行1∶1病例对照研究;应用PCR技术对鲍曼不动杆菌进行Ⅰ型整合子测定。结果研究显示鲍曼不动杆菌医院感染相关的危险因素依次为病情(OR=18.86)、人工气道(OR=8.47)、机械通气(OR=6.16)和抗菌药物使用天数(OR=5.41)。鲍曼不动杆菌Ⅰ型整合子分布特征明显,多重耐药菌株阳性率明显高于非多重耐药菌株。结论病情、人工气道、机械通气和抗菌药物使用天数是鲍曼不动杆菌医院感染危险因素;I型整合子作为鲍曼不动杆菌耐药性监测的工具,值得开展。Objective This article intends to investigate Acinetobacter baumanii in nosocomial infection and analyze the antibiotic resistance profiles of A. baumanii ; to determine the risk factors of the A. baumanii infections ; to investigate the distribution of Class I integron in A. baumanii;to discuss the associativity between A. baumanii and Class I integron. Methods Information about the 89 patients with A. baumanii hospital infection in 2007 was ob- tained respectively. To detect the distribution of Class I integron, PCR aided transcript titration assayas was employed to analyze the strains isolated from the 89 cases with A. baumanii infection. Results Multi - factor Logistic regression model indicats that the odds ratios ( ORS ) of A. baumanii are as follows : state of illness ( OR = 18.86 ), artificial air- way ( OR = 8.47 ), mechanical ventilation ( OR = 6. 16 ) and the course of antibiotic treatment ( OR = 5.41 ). The finding shows that lab rapid detection (PCR) of A. baumanii and Class I integron is very useful for the study on noso- eomial infection, bacterial drug resistance monitoring and Molecular Epidemiology. Conclusion The 1 : 1 case control study shows that related odds ratio of A. baumanii are state of illness, artificial airway, mechanical ventilation and the course of antibiotic treatment. The study also reveals that the distribution feature of Class I integron in A. haumanii is evident. And the positive rate of multi - resistant stains is much higher than that of nonmuhidrug - resistance Stains.
