曲安奈德实验性外伤性玻璃体出血的治疗效应

摘　　要：目的探讨玻璃体腔注射曲安奈德治疗外伤性玻璃体出血的效果。方法建立20只外伤性玻璃体出血家兔模型。随机分为曲安奈德组和生理盐水组,每组各10眼。直接眼底镜观察玻璃体出血指数、增生性玻璃体视网膜病变(PVR)程度;ELISA测定玻璃体液中细胞因子浓度。结果曲安奈德组PVR程度、玻璃体腔白细胞介素-1β(IL-1β)、表皮生长因子(EGF)、转化生长因子β1(TGF-β1)浓度低于生理盐水组(P<0.05)。曲安奈德组玻璃体出血指数、玻璃体腔基质金属蛋白9(MMP-9)、血小板源性生长因子(PDGF)与生理盐水组比较差异无统计学意义。结论玻璃体腔注射曲安奈德可下调玻璃体腔IL-1β、TGF-β1、EGF表达,抑制增殖细胞的迁移和增生,从而防治PVR形成和发展。Objective To study the therapeutic effect of triamcinolone for experimental traumatic vitreous hemorrhage.Methods After establishment of experimental model of traumatic vitreous hemorrhage in 20 New Zealand rabbits,which were randomly divided into two groups of triamcinolone（group T） and normal saline injection（group C） with 10 eyes each.The bleeding indexs in vitreous and degrees of proliferative vitreoretinopathy were estimated by direct ophthalmoscope examination.The density of cytokines in vitreous was detected by enzyme-liked immunosorbent assay.Results The degrees of proliferative vitreoretinopathy,density of interleukin-1β（IL-1β）,epidermal growth factor（EGF） and transforming growth factor-beta 1（TGF-β1） in vitreous were lower in group T than those in group C（P〈0.05）.There were no significant differences in bleeding indexs in vitreous,matrix metalloproteinase-9（MMP-9） and platelet-derived growth factor（PDGF） between two groups.Conclusion Intravitreal injection of triamcinolone can decrease the degrees of proliferative vitreoretinopathy through down-regulating the production of IL-1β,TGF-β1 and EGF in vitreous.
