摘 要:目的探讨睡眠呼吸暂停低通气综合征(sleep apnea hypopnea syndrome,SAHS)患者不同程度缺氧累积时间与日间嗜睡、体质量指数(body mass index,BMI)的相关性。方法对1093例拟诊SAHS患者进行多导睡眠监测(polysomography,PSG),分别统计睡眠呼吸暂停低通气指数(apnea hypopnea index,AHI)、平均血氧饱和度(mean oxygen saturation level during sleep time,MeanSaO2)、最低血氧饱和度(the lowest oxygen saturationlevel,LSaO2)、血氧饱和度低于0.90、0.80、0.70占监测总时间的百分比(the percent of the total recorded timespent below0.90,0.80,0.70oxygen saturation level,TS90%,TS80%,TS70%)、Epworth嗜睡评分(Epworth sleepi-ness scale,ESS)、BMI。将TS90%、TS80%、TS70%与ESS、BMI作Pearson相关分析。结果 ESS与TS90%、TS80%、TS70%呈正相关(r=0.324、0.259、0.186,P<0.01);BMI与TS90%、TS80%、TS70%呈正相关(r=0.521,0.432,0.364,P<0.01);LSaO2与ESS呈负相关(r=-0.220,P<0.01),LSaO2与BMI呈负相关(r=-0.399,P<0.01)。结论 SAHS患者缺氧累积时间与日间嗜睡、BMI密切相关,血氧饱和度下降累积时间越长,患者BMI越高,白天嗜睡越明显。(中国眼耳鼻喉科杂志,2010,10:154-156)Objective To explore the relationships between anoxia accumulation time and daytime sleepiness and body mass index in patients with sleep apnea-hypopnea syndrome (SAHS). Methods Altogether 1 093 patients with suspected SAHS were detected with polysomnography (PSG). Excessive Daytime Sleepiness was assessed with Epworth sleepiness scale (ESS). Apneahypopnea index (AHI) , mean oxygen saturation level during sleep time (MeanSaO2 ), the lowest oxygen saturation level ( LSaO2 ), the percent of the total recorded time spent below 0.90 oxygen saturation level (TS90 %, TS80% ,TS70% ) ,ESS and body mass index (BMI) was calculated. Pearson linear correlation was used to study the association between TS90% , TS80% , TS70% and ESS, BMI. Results There was a positive correlation between ESS and TS90% ,TS80% ,TS70% ( r = 0. 324,0. 259,0. 186 ; P 〈 0.01 ) respectively ; There was a positive correlation between BMI and TS90% ,TS80% ,TS70% ( r = 0. 521,0. 432,0. 364 ; P 〈 0.01 ). LSaO2 bad a negative relation- ship with ESS( r = -0.220,P 〈 0.01 ) or BMI (r = -0. 399,P 〈 0.01 ). Conclusions The anoxia accumulation time is closely correlated with daytime sleepiness and BMI in SAHS patients. The longer the oxygen desaturation accumulation time, the higher the BMI and the more severe the daytime sleepiness will be. (Chin J Ophthalmol and Otorhinolaryngol, 2010,10 : 154-156)
