摘  要:目的:探讨补阳还五汤注射对脑缺血再灌注损伤的治疗作用及机制。方法:88只雄性蒙古沙鼠随机分成对照组、模型组和补阳还五汤高剂量组(51.48mg/kg)、低剂量组(25.74mg/kg)。Kirino的方法制作沙鼠前脑缺血模型。术后1、6h,1d、3d、7d尼氏染色观察海马区神经细胞组织形态变化,免疫组化法和Western-Blot法检测海马区磷酸化p38MAPK和Caspase-3蛋白的表达,TUNEL法检测凋亡细胞,术后7~13d八臂迷宫法测试动物学习记忆功能。结果:与对照组比较,脑缺血后海马神经元结构损伤明显、磷酸化p38MAPK表达水平、Caspase-3蛋白表达、凋亡神经细胞数量增加;大鼠的习记忆功能下降;与模型组比较,补阳还五汤组中大鼠的习记忆功能得到改善、神经元形态结构损伤减轻、磷酸化p38MAPK蛋白、Caspase-3蛋白以及神经细胞凋亡数量回降,上述变化在高剂量补阳还五汤组更为明显。结论:补阳还五汤对脑缺血再灌注损伤有治疗作用,其机制与调控p38MAPK信号通路,抑制神经细胞凋亡有关。Objective:To investigate the effects of Buyanghuanwu recipe on orebrain ischemia-reperfusion and its underlying mechanism in gerbils. Method:Male gerbils (n=88) were divided randomly into four groups:control group,model group,low dose of Buyanghuanwu recipe treatment group and high dose of Buyanghuanwu recipe treatment group.Transient forebrain ischemia model was formed by bilateral common carotid artery occlusion according to the description of Kirino's method. At 1h,6h,1d,3d,7d after operation,changs of neuron morphous in hippocampal region were observed by Nissl staining;The expression p38MAPK phosphorylation and Caspase-3 were detected by immunohistochemistry and Western-Blot,and apoptotic cells was measured by TUNEL method. Learning and memory function (eight-arm maze) were performed daily from 7d after injury to 13d.Results:After operation,some neurons displayed changes of necrosis and apoptosis;the level of p38MAPK phosphorylation,the Caspase-3 expression and the number of neural apoptotic cells were significantly enhanced; learning and memory function was significantly decreased.After treatment with Buyanghuanwu recipe,the morphous injury,the p38MAPK phosphorylation levels,the Caspase-3 expression,the number of neural apoptotic cells wwere decreased obviously;and the ability of learning and memory function was enhanced.The above mentioned changes were more significant in high dose of Buyanghuanwu recipe group.Conclusion:Buyanghuanwu recipe have good therapeutic effect on after forebrain ischemia-reperfusion injury and the molecular mechanism is related to attenuate p38MAPK pathway and inhibite neuronal apoptosis.
