无创正压通气治疗老年社区获得性肺炎合并心力衰竭患者疗效分析

摘　　要：目的探讨无创正压通气(NPPV)对老年社区获得性肺炎(CAP)合并心力衰竭的治疗效果。方法老年CAP合并心力衰竭患者82例分成2组,观察组42例进行抗炎,利尿和扩血管等常规治疗,同时NPPV治疗,对照组40例只给与给与常规治疗。比较2组24h血气分析和脑钠素(BNP)区别。结果观察组和对照组治疗后PaO2分别为82.3±16.5和69.4±12.4mmHg(P<0.05),BNP分别为204.3±44.6和389.1±36.8pg/mL(P<0.05)。结论 NPPV对老年CAP合并心力衰竭患者有较好的治疗效果,可迅速提高动脉血氧含量,改善心功能。Objective To study the effect of noninvasive positive pressure ventilation for the treatment of community-acquired pneumonia in elderly patients with heart failure.Methods 82 elderly CAP patients with heart failure were divided into two groups,the observation group（n=42）were given anti-inflammatory,diuretic,vasodilator such as conventional treatment and NPPV treatment,the control group（n=40）were given conventional therapy alone.The difference of blood gas analysis and brain natriuretic peptide（BNP）between the two groups was compared after 24 h treatment.Results The PaO2 level of the observation group and the control group after the treatment were 82.3±16.5 and 69.4±12.4 mmHg respectively（P〈0.05）,BNP were 204.3±44.6 and 389.1±36.8 pg/mL respectively（P〈0.05）.Conclusion NPPV has a better therapeutic effect for elderly CAP patients with heart failure patients,it can rapidly enhance the arterial oxygen content and improve cardiac function.
