69例结核性毁损肺全肺切除术治疗的分析

摘　　要：目的总结全肺切除术治疗结核性毁损肺的临床效果。方法回顾分析我院近10年来行全肺切除手术治疗69例结核性毁损肺的临床资料,其中行右全肺切除术29例,左全肺切除术40例。结果 28例痰抗酸杆菌涂片阳性患者全部转阴。随访2个月～10年6个月。术后并发呼吸衰竭1例(死亡),心功能衰竭5例(死亡1例),心律失常38例,支气管胸膜瘘4例,血胸3例,脓胸3例,围术期死亡2例,其余均治愈。结论全肺切除手术治疗结核性毁损肺是治疗难治性肺结核的有效方法 ,但是手术难度高,风险大,并发症多,应做好防范手术风险和预防术后并发症的充分准备。Objective To summarize the clinical effect of pleuropneumonectomy treatment of tubercular with destroyed lung.Methods Clinical material of 69 cases with the destroyed lung pleuropneumonectomy near for 10 years were analyzed,29 cases with right pleuropneumonectomy,40 cases with left pleuropneumonectomy.Results 28 cases with sputum tuberculosis positive was sputum negative.A follow-up visit during 2 months to 10 year 6 months was made.After the surgery,1 case was with concurrent respiratory failure （death）,5 cases were with heart function failure（1 case died）,38 patients with arrythmia,4 cases with bronchial tube pleural membrane fistula,3 cases with hemothorax,3 cases with pyothorax,during the postoperative 2 cases died.Conclusion The pleuropneumonectomy treatment of tubercular with destroyed lung is efficacious,but the surgery difficulty is high,the risk is big,and the complications are many,we should prepare for the guard surgery risk and the prevention technique,complication full.
