摘 要:目的探讨阿魏酸钠对创伤性休克复苏兔胃黏膜细胞凋亡及其相关基因Bcl-2和Bax的影响。方法家兔60只随机分为对照组(A组)、模型组(B组)、复苏前(C组)和复苏后(D组),每组15只。建立创伤性休克复苏胃黏膜损伤动物模型,C组于复苏前20 min、D组于复苏30 min时缓慢静脉滴注阿魏酸钠30 mg/k,A组、B组静脉滴注等量生理盐水。采用原位末端标记(TUNEL)法检测胃黏膜细胞凋亡指数(AI),免疫组织化学法检测胃黏膜Bcl-2和Bax蛋白表达灰度值。结果与A组比较,B组AI值、Bcl-2蛋白表达灰度值及Bcl-2/Bax蛋白表达灰度值比显著升高,Bax蛋白表达灰度值明显降低(P<0.05或P<0.01)。与B组比较,C组、D组AI值、Bcl-2蛋白表达灰度值及Bcl-2/Bax蛋白表达灰度值比显著降低,Bax蛋白表达灰度值明显升高(P<0.05或P<0.01)。与C组比较,D组AI值、Bcl-2蛋白表达灰度值、Bcl-2/Bax蛋白表达灰度值比显著升高,Bax蛋白表达灰度值明显降低(P<0.05)。结论阿魏酸钠通过促进Bcl-2蛋白表达,抑制Bax蛋白表达,提高Bcl-2/Bax蛋白表达比值,抑制创伤性休克复苏胃黏膜细胞凋亡,且复苏前20 min应用效果较佳。Objective To investigate the effects of sodium ferulate on cellular apoptosis and expression of apoptosis related protein Bcl-2 and Bax in gastric mucosa after traumatic shock resuscitation.Methods Sixty healthy rabbits were randomly divided into four groups with 15 in each group:control group(group A),model group(group B),before resuscitation sodium ferulate trerapy group(group C),and after resuscitation sodium ferulate therapy group (group D).The traumatic resuscitation gastric mucosal injury model was established.In group C and D,sodium ferulate 30 mg/kg was injected intravenously 20 min before resuscitation and 30 min at resuscitation respectively.The drug was replaced with equal volume of physiological saline in group A and B.TUNEL technique was applied to examine the apoptotic index(AI) in the gastric mucosa cells.The gray values of Bcl-2 and Bax protein expression were detected by immunohistochemical assay.Results In group B,the AI and the expression gray of Bcl-2 and the expression gray ratio of Bcl-2/Bax were significantly increased,while the expression gray of Bax was significandy decreased compared with those in group A(P0.05 or P0.01).In group C and D,the AI and the expression gray of Bcl- 2 and the expression gray ratio of Bcl-2/Bax were significantly decreased,while the expression gray of Bax was significantly increased compared with those in group B(P0.05 or P0.01).In group D,the AI and the expression gray of Bcl-2 and the expression gray ratio of Bcl-2/Bax were significantly increased,while the expression gray of Bax was significantly decreased compared with those in group C(P0.05).Conclusion It demonstrated that the sodium ferulate could inhibit cellular apoptosis of gastric mucosa induced by traumatic shock resuscitation by up-regulating Bcl-2 protein expression and down-regualting Bax protein expression and increasing the expression ratio of Bcl-2/ Bax,and the remedial effects was better before resuscitation than after resuscitation.
