摘　　要：目的通过对输血申请单书写情况统计,分析书写不规范的原因,找到解决办法。引起医疗、护理人员对输血申请单书写的重视。方法对2009年3～12月输血科收到临床9660份输血申请单,逐项核查并统计,以项目完整、规范为合格。结果 9660份输血申请单中,规范书写的5258份,占54.4%,不规范书写的4402份,占45.6%,不规范书写累计达12803处之多。结论输血申请单的不规范书写可能为输血安全和医疗纠纷留下隐患。通过对输血申请单统计分析,找到其中的原因,提高医疗、护理人员对输血申请单规范书写的重视,确保临床输血安全,防范由此引起的医疗纠纷。Objective Through the statistical analysis of the filling of blood transfusion application forms,we in order to seek for the reasons for non-standard filling,to find out the solution as well as to cause a attention for the medical workers.Methods We have investigated 9660 transfusion application forms that collected from March 2009 to September 2009 of our department carefully,choosing the integrity and normal as a standard.Results 5258 pieces of 9660 are standard filled,accounting for 54.4%,while 4402 was non-standard filled,accounting for 45.6%.And we have discovered 12803 errors or non-standard filled in all theses forms.Conclusion Non-standard filled transfusion application form may be cause hidden trouble transfusion safety and health disputes.Through the statistical analysis for the transfusion application forms,we can find the error causes,and take measures to make the security of the clinical transfusion,also pay a attention for the medical workers.
