摘　　要：目的探讨Pilon骨折分类、治疗与影响疗效因素的分析。方法对32例Pilon骨折(按Rüedi-Allgower分型:其中Pilon骨折Ⅰ型6例,Ⅱ型14例,Ⅲ型12例)采用切开复位钢板内固定治疗,其中8例辅以外固定;20例给以植骨修补骨缺损,其中13例给以自体松质骨填塞,4例给以自体髂骨块植入,3例植入人工骨。结果自2005年3月至2009年2月,采用Burwell-Chamley系统对骨折复位评分,其中优10例,良18例,可1例,差3例。结论在Pilon骨折治疗中,自体松质骨、髂骨块植骨有利修补严重骨缺损,提高内固定稳定性,使关节早期活动,有利于功能恢复。Objective To discuss and analyze the effect of the Pilon fracture.[Method] Thirety-two patiens with Rüedi-Allgower type Ⅰ～Ⅲ Pilon Fractures（Ⅰ=6,Ⅱ=14,Ⅲ=12） were treated by open reduction and internal fixation.Eight cases were combined with extemal fixation. Twenty cases were treated by bone transplantation （ spongy bone=13,big pieces of ilium 4,artificial bone=3）.Result From March 2005 to February 2009 the restoration of the fractures with Burwell-Chamle system was evaluatet： excellent=10,good=18, fair=1,bad=3.Conclusion in the time of treating Pilon fractures, the bone transplantation with spongy bone and big pieces of ilium is effective to solve the seirious bone lose, strengthen the internal fixation and execies ankle joint earlier.
