乙肝母亲HBV DNA含量及肝功能与其新生儿HBV宫内感染的关系

摘　　要：[目的]探讨乙型肝炎母亲血清HBVDNA含量及反映肝脏损伤的相关肝功能指标与新生儿HBV宫内感染的关系。[方法]连续收集2006年9月~2007年7月在太原市传染病医院产前检查及分娩的191例HBsAg阳性孕妇及其新生儿为研究对象,用荧光定量PCR（FQ-PCR）法检测母亲及其新生儿血清HBVDNA含量;用全自动生化仪检测肝功能;酶联免疫吸附试验（ELISA）检测新生儿血清HBsAg。采用SPSS13.0统计软件进行统计分析。[结果]孕妇血清HBVDNA含量与HBV宫内感染有关,且HBVDNA含量≥104copy/ml以上与宫内感染显著相关;乙肝母亲丙氨酸氨基转移酶（ALT）、天冬氨酸氨基转移酶（AST）水平与HBV宫内感染无关联。[结论]孕妇血清HBVDNA含量是HBV宫内感染的危险因素,且HBVDNA含量≥104copy/ml时,宫内感染发生率显著增加;乙肝母亲肝功能异常与HBV宫内感染无关。[Objective] To explore the relations between HBV intrauterine infection and the contents of HBV DNA, the index of liver function in HBsAg positive mothers. [Methods] 191 pregnant women and 191 newborns were collected between September, 2006 and July, 2007 in Taiyuan infectious hospital. HBV DNA contents in blood of pregnant women and their newborns were detected by real-time fluorescence quantitative PCR. Liver function was determined with automatic biochemical analyzer. [Results] The contents of HBV DNA in HBsAg-positive pregnant women had statistical significance between case group and control group. When the maternal serum HBV DNA level ≥104 copies/ml, the two groups had a significant statistical difference. Whether the liver function of pregnant women was abnormal. [Conclusion] The grade of HBV DNA level ≥104 copies/ml, serum HBV DNA level is the risk factor for intrauterine HBV infection, and for those with serum the chance for intrauterine HBV infection can be significantly increased. The research does not find whether the liver function of pregnant women abnormally is concerned with intrauterine infection.
