ELISA方法S/CO值应用于乙肝表面抗体保护范围初探

摘　　要：[目的]探求乙肝表面抗体(HBsAb)ELISA法测定的S/CO比值对乙型肝炎病毒(HBV)感染有保护意义的范围,以便让受检者了解自己产生的HBsAb是否有保护作用,是否需加强免疫。[方法]用ECLIA和ELISA两种方法同时测定306例受检者血清的HBsAb与乙肝表面抗原(HBsAg)。[结果]ECLIA法测HBsAg为阴性207例标本中,如ELISA法的S/CO值﹤1.0(n=67),ECLIA法则有58例小于10mIU/ml,一致性86.6%;9例大于10mIU/ml,占13.4%;无1例大于100mIU/ml。如S/CO值介于1.0～5.0(n=32),8例小于10mIU/ml,占25.0%;22例在10～100mIU/ml之间,一致性68.8%;2例大于100mIU/ml,占6.3%。如S/CO值介于5.0～13.0(n=31),无1例小于10mIU/ml;20例在10～100mIU/ml之间,占64.5%;11例大于100mIU/ml,占35.5%。如S/CO比值大于13.0(n=77),则有76例大于100mIU/ml,一致性高达98.7%,其中有37.7%大于1000mIU/ml,仅1例小于100mIU/ml,其定量值为88.4mIU/ml。表面抗原为阴性且ECLIA定量为5～300mIU/ml,两方法学之间的回归方程为y=0.0871x+0.0978(r=0.954,P﹤0.001);HBsAb定量ECLIA法为100mIU/ml相应的ELISA法的S/CO比值为8.808、95%的可信区间为8.364～9.251及95%的预测区间为5.012～12.603。[结论]与ECLIA方法相比受检者的HBsAb具有肯定的免疫保护作用的S/CO比值为大于13.0。[Objective] To explore the S/CO ratios of protection significance scope of Hepatitis B Virus antibody （HBsAb） with ELISA to the hepatitis B virus （HBV） infection, and to provide reference information on whether HBsAb has protective function or not, then to decide to strengthen immunization oneself. [Methods] The HBsAb and hepatitis B surface antigen （HBsAg） of 306 serums were detected with ECLIA and ELISA. [Results] In the 207 HBsAg-negative samples, the S/CO ratios were below 1.0 in HBsAb with ELISA （n = 67）, 58 cases were lower than 10 mIU/ml （86.6%）, 9 cases were more than 10mIU/ml with ECLIA （ 13.4% ）. If the S/CO ratios were between 1.0-5.0 （ n = 32） , 8 cases were less than 10mIU/ml （25.0%）, 22 cases in the 10-100 mIU/ml （68.8%）, only 2 cases more than 100mIU / ml （6.3%）. If the S/CO ratios were between 5.0-13.0 （n = 31）, 20 cases in the 10-100mIU/ml （64.5%）, then 11 cases more than 100 mIU/ml （35.5%） and no one below than 10 mIU/ml; If the S/CO ratios were more than 13.0 （n = 77） , 98.7% more than 100mIU/ml, including 37.7% more than 1 000 mIU/ml, and only one sample was lower than 100 mIU/ml, the value was 88.4mIU/ml. In 5-300 mIU/ml of the ECLIA quota and HBsAg-negative, HBsAb of the regression equation between ELISA and ECLIA was y=0.0871x＋ 0.0978 （r = 0.954, P﹤0.001）. The S/CO ratio, 95% Confidence Interval and 95% Prediction Interval of HBsAb quota in 100 mIU/ml was 8.808, 8.364-9.251, 5.012 -12.603, respectively. [Conclusion] The S/CO ratios of protection significance scope of HBsAb with ELISA to the HBV infection is more than 13.0.
