老年胆囊切除术患者诊治体会

摘　　要：[目的]探讨老年胆囊切除术后并发症的特点及围手术期的处理。[方法]对1996年1月~2008年12月行胆囊切除术的618例老年患者,与同期施行胆囊切除术的401例中青年患者在术后并发症发生率及引起并发症发症的相关危险因素等方面进行比较。[结果]胆囊切除术后老年组并发症发生率为17.31%,中青年组为10.22%,老年组胆囊切除术后并发症发生率高于中青年组（χ2=8.7,P﹤0.05）;直接与手术有关的并发症老年组发生率为6.23%,中青年组为6.31%,两组差异无统计学意义（χ2=0.006,P﹥0.05）;间接与手术有关的并发症发生率老年组为11.00%,中青年组为3.90%,提示胆囊切除术后间接手术并发症发生率老年组较中青年组增高（χ2=14.9,P﹤0.05）。[结论]老年患者胆囊切除术后并发症发生率高于中青年患者,应加强老年患者围手术期的处理,减少并发症的发生率;对有手术机会的老年患者应积极给予手术治疗,提高老年患者的生存质量。[Objective] To study the characteristics of the complication after cholecystectomy and treatment in perioperative period in elderly patients. [Methods] Comparison on the complication and the related risk factors were analyzed among the 618 cases of elderly patients and the 401 cases of young and middle aged patients with cholecystectomy. [Results] Incidence rate of complication in the elderly patients group （17.31%） was significantly higher than that of the young and middle aged patients group （10.22% ） （P﹤0.05）. Incidence of directly related postoperative complication in the elderly patients group was 6.23% and in the young and middle aged patients group was 6.31% .No significant difference was found （P﹥0.05）. Incidence of indirectly related postoperative complication rate in the elderly patients group （11.00%） was significantly higher than that of the young and middle aged patients group （3.90%）（P﹤0.05）. [Conclusion] Incidence of complication in the elderly patients group is higher than that of the young and middle aged patients group. It should strengthen treatment of the elderly patients in perioperative period to reduce the complication. It should treat the elderly patients who are suitable with operation to improve survival quality.
